An example of a hard-to-heal wound is a pressure ulcer, otherwise known as bedsore. These form on bony prominences, usually in cases where people are immobilized for extended periods of time such as people who are injured or the elderly.
The factors discussed include oxygenation, infection, age and sex hormones, stress, diabetes, obesity, medications, alcoholism, smoking, and nutrition. A better understanding of the influence of these factors on repair may lead to therapeutics that improve wound healing and resolve impaired wounds.
Remodeling or also known as maturation phase is the fourth and final phase in wound healing and lasts from 21 days up to 2 years. In this final and longest phase, collagen synthesis is ongoing in order to strengthen the tissue. Remodeling occurs as wound continues to contract and fibers are being reorganized.
Remodelling is the final phase of the healing process in which the granulation tissue matures into scar and tissue tensile strength is increased (Figure 23.5).
A chronic wound is a wound that does not heal in an orderly set of stages and in a predictable amount of time or wounds that do not heal within three months are often considered chronic. Chronic wounds often remain in the inflammatory stage for too long and may never heal or may take years.
Avulsion. Also known as a skin tear, an avulsion is a deep break to the skin, tearing it away from the underlying tissue. Wounds of this type are usually more serious and extensive.
However, more severe cuts or wounds – also called lacerations – require medical attention. Uncontrolled bleeding is an immediate risk; a longer-term concern is wound infection, which, if untreated, can lead to sepsis and turn life- or limb-threatening. Injuries of this type can also prove painful.
Avulsions are severe injuries that can cause uncontrolled, life-threatening bleeding. This type of injury typically occurs when skin or a body part is partially or completely torn away.
Diabetes can slow the healing process by making white blood cells less effective. Diabetes, cardiovascular disease, and other conditions that give rise to poor circulation may also make it harder for your body to deliver oxygen, nutrients, and reparative cells to an injured area.
A handful of studies have found that when wounds are kept moist and covered, blood vessels regenerate faster and the number of cells that cause inflammation drop more rapidly than they do in wounds allowed to air out. It is best to keep a wound moist and covered for at least five days.
Wound bed. Healthy granulation tissue is pink in colour and is an indicator of healing. Unhealthy granulation is dark red in colour, often bleeds on contact, and may indicate the presence of wound infection. Such wounds should be cultured and treated in the light of microbiological results.
Wounds generally heal in 4 to 6 weeks. Chronic wounds are those that fail to heal within this timeframe. Many factors can lead to impaired healing. The primary factors are hypoxia, bacterial colonization, ischemia, reperfusion injury, altered cellular response, and collagen synthesis defects.
Once the wound has formed a scab, there is no longer the need to cover it with a bandage as the scab now acts as a protective barrier. Keep the area clean, but be gentle so that you do not accidentally remove the scab.
Itching is a natural part of the body's healing process. When the skin is injured, the body sends blood cells to the area to repair the damage. These cells release substances that cause inflammation and itchiness, a sign that the healing process is underway.
Signs of Healing
Your general cuts and puncture wounds go through three stages of healing: bleeding, clotting, and scabbing. If you find your wound is still bleeding after a significant period and no scab is forming, you may need to seek further care.
Keep your wound covered with clean gauze or an adhesive bandage during waking hours. You can leave it uncovered while you sleep if it isn't oozing or painful. Don't soak your wound for long periods when bathing.

Infection. A common cause of delayed wound healing is infection. At the time of injury, microorganisms can enter the tissue. These microbes can delay wound healing by further prolonging the inflammatory process.
What Part of the Body Heals the Slowest? Ligaments, nerves and wounds in areas with more movement heal the slowest. Injuries to these areas have a longer recovery time because of poor blood circulation and constant motion stress.
