Tom Riddle becomes so disfigured primarily because of his Horcruxes. Before his enterprising Dark Arts work, nobody had ever made more than one Horcrux. Voldemort made seven.

The Horcrux affects Hermione the least of the trio, but it does affect her. Harry holds up pretty well while wearing it also. Outward appearance
After he regains his body in the fourth book, Rowling describes Voldemort as having pale skin, a chalk-white, skull-like face, snake-like slits for nostrils, red eyes and cat-like slits for pupils, a skeletally thin body and long, thin hands with unnaturally long fingers.
The most likely reason why Lord Voldemort's nose disappeared and he devolved into such an evil-looking creature is that as he dabbled deeper into the Dark Arts, his appearance gradually became as distorted as what remained of his twisted soul.
Voldemort wasn't aware of Harry being a pseudo-horcrux because he didn't plan it, and Harry didn't know either until Voldemort “killed” him, but he actually killed the piece of soul kept in him.
Green is the colour of much 'Dark' magic; of the 'Dark Mark', of the luminescent potion in which Voldemort conceals one of his Horcruxes, of many 'Dark' spells and curses, and of Slytherin house.

Phoenix tears are not a cure all. They are a powerful healing agent for wounded flesh, poison, physical damage and disease. But Dumbledore's hand was not affected by any of these, it was affected by a powerful magical curse that was slowly and steadily spreading. Phoenix tears are not a counter spell.
Harry proceeds to tell Delphi that she will always be an orphan and they return to the correct time, where Delphi would be taken to Azkaban. After Voldemort's death in 1998, Delphi became Salazar Slytherin's last known heir.
