Yoriichi Tsugikuni was given the small wooden flute by his older twin brother, Michikatsu Tsugikuni.
Michikatsu first crafted the wooden flute and gave it to Yoriichi when he was 7 who then kept the flute in remembrance of him until he died. Then Kokushibo split Yoriichi's corpse in half and saw the flute, having been carried by Yoriichi for all of his life.
Yoriichi and Kokushibo were twins. Tanjiro's ancestor and Yoriichi were friends (hence why they learned sun breathing). So Yoriichi and Kokushibo were not related to the Kamado clan at all.

After falling into a coma in the aftermath of his battle against Upper Rank Six, Tanjiro Kamado dreams of memories belonging to his ancestor Sumiyoshi. Within them, Yoriichi is seen holding a newborn Sumire as her mother sleeps.

muichiro is only descendant to side of michikatsu (kokushibu). yoriichi don't have any descendant from his side. if you're saying muichiro is still a descendant to yoriichi because kokushibu are twin, no it's not. the working system of a twin have slight differences.
