While Arthur struggles in his job as a clown-for-hire, he falls in love with a pretty neighbor, a single mother named Sophie (Zazie Beetz). One of Joker's big twist occurs when Arthur's “dates” Sophie only to find out that those flirty conversations and their eventual hookup were completely in his imagination.

Character traits
He longed for a romantic relationship throughout his life, that he seemed to never have gotten. He had a compassionate mother/ son relationship with his mother Penny, until he found out the latter had lied to him and abused him as a child.
While in Arkham City, Batman was ambushed by Clayface (disguised as Joker) and knocked out by Harley Quinn. After he woke up, Batman discovered that Joker suffered from a horrible side effect of Titan which gave him a fatal disease that progressively killed him.
While Penny cares about her son, she has shown to lie to him multiple occasions which eventually leads to her downfall once Arthur finds out the truth. Penny is also shown to be somewhat degrading to Arthur at times, claiming that he "needs to be funny" in order to be a comedian.
Harley and Joker break up because Harley is sick of living in Joker's shadow. She was more intelligent than the Joker and often planned their major heists. However, Joker always took all the credit.

Although Joker is never diagnosed with any specific psychological disorder in the film, his symptoms indicate that he has delusional disorder, bipolar disorder, and pseudobulbar affect.
On the surface, this Joker wants to look “happy.” That's what he's told he's supposed to be. And, his uncontrollable laughing condition is because of how he's been forced to control his anger and sadness; to “stuff it” deep down inside.
