There is no allowance for mistakes on an official passport form; accuracy is critical as errors can lead to delays, rejection, or potentially even legal penalties.
If you do not meet these criteria, you willl need to lodge your application in person, by prior appointment only. Please do not use whiteout on the declaration section. If you make a mistake, you can cross it out and initial next to the change. If you have signed incorrectly, please fill in a new form.
I have printed my online form and have found a mistake. How can I correct it? At the Passport Seva Kendra (PSK)/Post Office Passport Seva Kendra (POPSK) please ask the Citizen Service Executive (CSE) i.e. at Counter-A to make the required changes in the application form.
Simply fill out a form DS-5504, sign a statement declaring the error, and submit proof of the correct information, such as a birth certificate showing your correct date of birth or the correct spelling of your name.
U.S. Passport Denial and Revocation
If the application is denied, the Department of State will send the applicant a written notice setting forth the reasons for the denial and procedures for review. Within 90 days, the applicant can submit documentation to overcome the denial without a new application.

Some reasons to deny a passport include: A valid, unsealed federal warrant of arrest. A federal or state criminal court order. A condition of parole or probation forbidding departure from the United States (or the jurisdiction of the court)
If you report the issue in time, the Passport Office will usually ask you to send both the new passport and your previous expired passport back to them for review. This applies whether you've made a mistake on your online passport application or the error was made by the Passport Office.
If your application has not yet been submitted online, go back to the relevant step, correct the error, and proceed. If it has already been submitted, contact the live chat support with your application reference and a description of the correction needed.
Appointment Rescheduling/Cancellation for an application is allowed up to two times only.

If your passport photo was rejected, you're not alone – and it's fixable. Most rejections occur due to common, avoidable mistakes like incorrect sizing, poor lighting, or improper facial expressions. The good news is that you'll be given an opportunity to resubmit a compliant photo within a specified timeframe.
Also, every applicant is given opportunity to verify particulars at Counter-A of PSK/POPSK. However, if you find any mistake/error in the particulars printed in the Passport booklet as per the application form submitted, please return the Passport for necessary rectification at Regional Passport Office concerned.
The US State Department advises you to take your picture in “clothing normally worn on a daily basis” in darker colors and without patterns. Plain t-shirts and collar shirts are appropriate, but uniforms and certain accessories (e.g., a hat) aren't allowed.
Yes. In most cases when you renew by mail, we will return the old passport to you. The old passport may not arrive for up to four weeks after you receive your new passport. If you renew online, keep your most recent passport and do not send it to us.
Please do not use correction fluid or similar products on a passport application form as it will invalidate that section of the form and you will have to complete another form. Instead, just cross out the incorrect information and sign next to it. Then correctly complete the section.
If you've failed to make payments on a loan from the Department of State you would need to pay it off before sending in your application. Certain court orders, being convicted of a felony, or if you're wanted on an arrest warrant, can also result in a passport denial.
Dues Not Paid
There is a good possibility that the passport application will be rejected if you are in debt, have availed substantial loans, or have borrowed on credit cards and failed to make consistent repayments.

A decision not to issue a passport or travel document is generally not considered an exceptional circumstance. We don't refund portions of application fees.
Your application will be rejected and you won't get a refund if: we withdraw your application because we haven't received all the information we've asked for. you're not entitled to a passport. you miss your appointment - you'll need to withdraw your application, apply and pay again.
Non-Refundable Fees
