To get the Mad Pumpkin Head (helm), you need to defeat the Mad Pumpkin Head mini-boss in the Waypoint Ruins cellar in Limgrave, Elden Ring; he drops the helm randomly, so you might need to farm him, but for the summonable Mad Pumpkin Head Ashes, you defeat the Erdtree Burial Watchdog Duo in the Minor Erdtree Catacombs in Limgrave.

Jack o'lanterns can spawn within taiga and snowy taiga villages in the piles of pumpkins that also generate there.
