Cora didn't get Aurora's heart directly; instead, Captain Hook took it from Sleeping Beauty and presented it to Cora as a gift, allowing Cora to control Aurora like a puppet, but Hook later returned the heart to Mulan to help the heroes, freeing Aurora from Cora's control in the Enchanted Forest. Regina had enchanted Hook's hook to remove hearts, but he used it on Aurora, not Cora, to gain leverage and later as a bargaining chip, demonstrating his shifting loyalties.
How did Hook manage to rip out Aurora's heart when he's never shown magical acumen before? Because Regina enchanted his appendage so that he could remove just one heart — and though she intended that heart to be Cora's, Hook didn't use his gift until after he had gone to Wonderland.
Gold leads Mary Margaret to find that he has the magical candle that Cora had given her years earlier, which can save a life by causing another person's death. He asks her to use it to save him, and she agrees once she realizes she can use the candle to kill Cora.
Before the evil duo can jump into the lake/portal, Snow and Emma and Mulan get there just in time. Mulan blocks a fire ball from Cora, Snow White ues her bow and arrow, and Emma takes the fight to Hook.
