Sanemi's unsocial, hateful personality is not just an act, he's also traumatized by his life which has been constant violence and has partly lost the ability to become close to others or speak his true feelings because those emotions are so painful for him.

Giyuu and Sanemi died because of the mark at 25 years old, other Hashiras died during the fights. Tanjirou did NOT die - he lived a long and happy life because the curse of the mark was reversed, so we can kinda say that Muzan saved him by making him a demon even tho it was only for a short time.

Kanae Kocho
Not much is currently known about Kanae and Sanemi's relationship. Gyomei Himejima claims that Sanemi likes Kanae, although we do not know if these feelings were reciprocated or even if they were romantic or platonic.
