Veronica couldn't stand the thought of the child being forgotten, so she agreed to adopt her. Immediately after Spencer's birth, a nurse gave Spencer to a man who then gave her to Veronica, who was waiting outside the sanitarium in a car. Spencer was never told that she was adopted.
The scene where Veronica revealed all of this to Spencer was heartbreaking. In her mind, there was no doubt that Spencer was her child even though they were not biologically related; she loved Spencer and would protect her at all costs. The big reveal, of course, was the identity of Uber A. It turns out that Spencer (Troian Bellisario) had an evil twin this entire time, and that "A.D." refers to her initials — Alex Drake.

She is Spencer and Charlotte's biological cousin as Alison's aunt, Mary, is Spencer and Charlotte's biological mother.
A streamlined version of Mary's story: She was a patient at Radley 25 years prior, where she gave birth to Charles Drake (who later transitioned to Charlotte, a.k.a. CeCe Drake) but was deemed unfit to be a parent. Thus Charles was adopted by Jessica and Kenneth DiLaurentis.
