In Doctor Strange in the Multiverse of Madness, Wanda Maximoff (Scarlet Witch) seemingly dies by crushing herself under the collapsing rubble of Mount Wundagore, after destroying the Darkhold's castle to stop her own multiversal corruption. While a definitive "killer" isn't present, she was the cause of her own demise, triggered by the Darkhold's influence and her desperate attempt to contain it by destroying the source, leading to a red energy burst and her apparent death under the falling rocks.
