The entity who takes the soul varies by belief, but commonly it's the Angel of Death, Azrael, in Abrahamic faiths (Islam, Judaism, some Christianity) who acts as a psychopomp to guide souls to the afterlife; in Greek mythology, Hermes Psychopomp guides souls; and in various folklore, "soul eaters" or other entities might consume souls.
He is responsible for taking the souls of the deceased away from the body. Azrael does not act independently, but is only informed by God when the time is up to take a soul.
