Because he was not an active member of the Jedi Order during Order 66, Infil'a escaped execution. Some time later, he faced the Sith Lord Darth Vader wielding a green lightsaber. Infil'a put up a fierce fight against Vader, almost killing him by throwing the Sith Lord off of a cliff.
Admiral Ozzel commanded the Super Star Destroyer Executor, Darth Vader's mighty flagship and the linchpin of the squadron searching the galaxy for the Alliance's secret base. Ozzel led the initial attack on Hoth, but his choice of tactics infuriated Vader, leading to a sudden and permanent demotion.

When Vader strangled Ozzel, Piett was granted a battlefield promotion to admiral -- and given a grisly warning of the penalty for failure. Miraculously, he survived his time as Vader's underling, but died when a Rebel starfighter destroyed the Executor's command bridge.
After the destruction of THE DEATH STAR, the disgraced Sith Lord, DARTH VADER, was demoted by his Master, Emperor Palpatine.
Vader gets the better of them and uses the Force to choke the guards and Amedda, but Palpatine in turn Force chokes Vader, lifting him off the ground, and then uses the Force to crush him as Palpatine hurls taunts at him. There are several parallel hand removal scenes in the Star Wars saga, but Luke cutting off Darth Vader's hand in "Return of the Jedi" is the most obvious.
During the Battle of Umbara, Jedi General Pong Krell was revealed to be a traitor to the Galactic Republic, having deliberately attempted to get as many of the clone troopers under his command as possible killed in an attempt to become the Sith apprentice of Count Dooku.

To escape the armor's claustrophobic nature, Vader had several pressurized meditation chambers built where he could remove his mask and suit and still survive.
In Star Wars Legends and Canon, Galen Marek (Starkiller) is the only Jedi (other than Obi-Wan Kenobi) to put a beat down on Darth Vader/Anakin Skywalker. Galen was secretly trained as a Sith Lord apprentice to Vader, but returned to the light side and the Jedi.
This information comes to light form the comic series Darth Vader: Dark Lord of the Sith by Charles Soule and Giuseppe Camuncoli, in particular issue #7. Palpatine admits to Darth Vader that the Jedi he actually feared most was none other than Jocasta Nu.

Darth Vader's Suit Embeds Needles In His Skin
Inside his suit and mask, Vader has a series of needles in his skin. Unfortunately, these needles, which transmit neurological data throughout his body and allow him to control his limbs, also cause him constant pain.
So, all of his limbs are, in fact, intact. However, Kylo Ren didn't walk out of that fight squeaky clean. We do know that Kylo is sporting a brand-new scar, which makes him even more like his granddad Darth Vader.
Erv Lekauf was a lieutenant in the Imperial Army who served as Darth Vader's personal aide for several years. On a mission to Vohai in 19 BBY, Lekauf helped Vader fight the Emperor's Hand Sa Cuis, impressing the Dark Lord with his courage and loyalty.
When he was Anakin, he feared losing the people closest to him, whether it was by distance or death. He did end up losing pretty much everyone close to him, namely Qui-Gon, his mother Shmi, then lastly Padme. It was his fear of never ending loss that drove him to become Vader.
Eventually, he lost both his legs and his remaining arm in the climactic duel on Mustafar between both he and Obi-Wan. As Vader, he had each limb replaced with cybernetic parts, but would lose his robotic right arm one last time when Luke Skywalker faced his father in Return of the Jedi.
Between Darth Vader and Obi-Wan, it's pretty clear that Vader is far more powerful with the force. After all, he pretty easily used the force to stop a starship from taking off in the previous episode, and manhandled Kenobi throughout the majority of the show.
Visions of Obi-Wan. The comic opens during the last moments of Revenge of the Sith, recreating the well-memed instance when a despondent, freshly-armored Vader cries out with a drawling “nooooooo” over how much he's lost.
During the Imperial Era, a human female nurse served the Galactic Empire on board the Death Star under the supervision of a doctor. They both served the Sith Lord Darth Vader as his personal medical caretakers. The nurse became obsessed and psychotically in love with the Dark Lord of the Sith.
