Nonetheless, Jamie did have sexual relations with Laoghaire in a bid to make the marriage work but ultimately failed and so the couple separated. Additionally, it seems Jamie was acting of out duty to his new wife however past trauma meant Laoghaire was unable to reciprocate.
In Outlander season 3, once Claire and Jamie are reunited back in the past, Jamie soon thereafter confesses his tryst with Geneva, explaining that he's produced a son out of the encounter, and yet, throughout his story, Claire remains reserved and understanding.
Therefore, she kept it a secret in order to protect Jamie's feelings. Though Claire was able to forgive Jamie for his infidelity with Laoghaire in the past, she was also aware that the situation was different in this case, as she was the one who had committed the unfaithful act.

It's Tom who eventually finds out. Malva, before her death, tells him that she made them sick. She'd collected parts of the dead Sin Eater and poisoned Claire and Tom. She wanted to get to Jamie, and getting Claire out of the way was the only way to do that.
Later, after Claire has returned to the Ridge, she is confessor to Allan Christie, who explains that he was the one that fathered Malva's child, and then murdered her.

Additionally, Allan confirmed he took Malva's life because she was going to reveal the truth and he didn't want this. Allan planned to take his life with Claire trying to dissuade him but Young Ian (John Bell) killed him with an arrow.
Malva was trying to bump off Claire in the hopes Jamie would take pity on the unwed mother and perhaps take her in. There's also a sense Malva was jealous of Claire and may have wanted her out of the picture to improve her own prospects.
When Claire returns to Scotland in the 17th century, she eventually becomes stepmother to Jamie's son by Geneva Dunsany, William Ransom, and his stepdaughter, Marsali MacKimmie Fraser, from his marriage to Laoghaire MacKenzie. She is later married to Lord John Grey.
