By measuring a woman's baseline FSH on day 3 of the cycle (we do it on day 2, 3, or 4), we get an indication as to whether she has normal “ovarian reserve”. We are looking at how hard her body needs to “step on the gas” early in the menstrual cycle to get a follicle growing.
FEMALE REPRODUCTIVE HORMONES
The hormonal peaks have been found to occur in the morning for progesterone, in the afternoon for FSH and LH, and during the night for oestradiol. These findings are unlikely to alter the time of day we order female reproductive hormone measurements.
Follicle-stimulating hormone (FSH) screening
The level of FSH in the body can fluctuate during the menstrual cycle and therefore FSH levels are usually tested on day 2 or 3 of a menstrual period.
Factors affecting FSH levels
These include: Age: Levels naturally rise, especially after age 35. Menopause: FSH levels increase significantly as ovarian activity ceases. Hormonal disorders: Conditions like PCOS or pituitary dysfunction can alter levels.
In most cases, you do not need to fast before a female hormone blood test. Hormones like oestrogen, progesterone, FSH, and LH are not immediately affected by food intake, meaning eating beforehand does not interfere with results.

WHAT IS A NORMAL FSH LEVEL FOR WOMEN?
The FSH level is actually fairly easy to lower medically (with estrogen, the birth control pill, Lupron, etc.), but the underlying problem (diminished ovarian reserve) that causes the elevated FSH cannot necessarily be “fixed.”
Generally, in women and men: Higher levels of FSH are often a sign of a condition in the reproductive glands (ovaries or testicles) that prevents them from making normal levels of sex hormones. The pituitary responds by making more FSH to try to get them to work properly.
Preparation: If you are still menstruating, it is recommended to test on day 3 of your menstrual cycle (cycle day 1 is the first day of your period). If you have no menstrual cycle or have irregular menstrual cycles, you can test at any time.
The ideal time to sample blood for FSH levels is on day 3 of a cycle although day 2-5 will probably suffice. Any testing outside of these days is irrelevant. When used as a screening test for ovarian reserve, levels of estradiol should also be measured. A combination of high FSH and high estradiol levels are bad news.

No fasting is required for FSH testing. Discontinue biotin supplements at least 72 hours before your blood draw to prevent assay interference (Mayo Clinic Labs 2023). Women should coordinate timing with their menstrual cycle for optimal accuracy.
It may be preferable to do an early morning sample for TSH in the latter two settings where narrower and stricter reference ranges for TSH are being advocated recently.
