The hook effect happens when extremely high levels of the pregnancy hormone hCG overwhelm a urine pregnancy test, causing it to give a false-negative result, often in late first trimester pregnancies, or with multiple babies (twins, triplets) where hCG is much higher, or with certain medical conditions like a molar pregnancy, usually when hCG exceeds 500,000 mIU/mL. It's rare, but can also occur with fertility treatments or certain hCG variants.
This typically occurs later in pregnancy or with multiple pregnancies when hCG levels become so elevated that they overwhelm the test's detection mechanism. The hook effect is most common after 8-10 weeks of pregnancy or in cases of twins or triplets where hCG production is significantly higher than normal.
This phenomenon is known as the "hook effect," and it's very rare. Dr. Caponero says that research suggests that it occurs in less than 2% of negative pregnancy tests.
In positive uhCG POC tests, free and fixed antibodies bind hCG to form a "sandwich"; hCG is present in several variant forms that change in their concentrations at different stages of pregnancy. When in excess, intact hCG can saturate the antibodies, preventing sandwich formation (hook effect phenomenon).
“While there's no exact minimum hCG level required, it typically occurs when levels reach 500 to 1,000 mIU/mL or higher, depending on the sensitivity of the test,” explains Peña. “Since hCG rises rapidly in early pregnancy, some people may see a dye stealer as early as 21 to 28 days post-ovulation, but this varies.”
At serum levels of hCG above 500,000 mIU/mL, a “hook effect” can occur, resulting in an artificially low or negative value when using the current commercially available immunometric hCG assays (Yeung and Cheung, 2014, Mori and Lurain, 2015).
In rare cases, hCG levels are so high that they can essentially overwhelm a pregnancy test, leading to a false negative result due to the hook effect. How high are we talking? The hook effect can occur when hCG levels are above 500,000 mIU/mL. The hook effect in pregnancy tests is rare.

In a twin pregnancy, hCG will most likely increase at a similar pace to a single pregnancy. The only difference is that the base level for a twin pregnancy will likely be much higher. This means that in the first four to six weeks after implantation, you can expect hCG to double every two days.
Typical hCG levels by gestational age are approximately: 3 weeks: 5-50 mIU/ml. 4 weeks: 5-426 mIU/ml. 5 weeks: 18-7,340 mIU/ml.
In a twin pregnancy, there are elevated levels of hCG, making it possible to have a very early positive result. However, home pregnancy tests won't confirm if twins are present, only the presence of hCG. It takes around 2 weeks after conception for hCG to be detected in a hCG pregnancy test.
As mentioned above, typically, methods of overcoming the hook effect require the use of more assay materials through methods such as serial dilution of the sample18 or the addition of one or more extra test lines to the strip6.
HCG can be found in your urine 5 to 7 days after conception or about 26 to 36 days after the first day of your last menstrual period. Except for very early in a pregnancy, this test can show if you are pregnant within days of a missed period.
