Infant botulism
Signs and symptoms of infant botulism may begin between 3 and 30 days after the toxin gets into an infant's body. If infant botulism is related to food, such as honey, problems generally begin within 18 to 36 hours after the toxin enters the baby's body.
Only give honey to healthy children over one year of age. By this age, children have developed helpful bacteria in their intestines that protect against Clostridium botulinum spores. Therefore, their risk of developing infant botulism is very low.

Babies with infant botulism (BAH-chuh-liz-im) can have muscle weakness, a weak cry, and trouble breathing. They need to be treated in a hospital. With early diagnosis and proper medical care, a baby should fully recover from the illness.
But infant botulism happens because bacteria are making the toxin inside your baby's body. That's why infant symptoms are usually slow to appear and only get worse gradually, compared to sudden, severe symptoms that are more likely in adults.
It can come from foods e.g. honey or poorly prepared meat (50% are type A), or from wounds (80% are type A). Often babies with infantile botulism have had honey ingestion. Many cases have no known cause, though, and are thought to be due to the inhalation of spores from dust.
Can babies have honey? The American Academy of Pediatrics (AAP) recommends [1] babies under 12 months should not eat honey, raw or cooked. Introducing honey to infants too soon puts them at risk of a serious, though rare, infection called botulism.

Bacterial spores in food are destroyed by high temperatures obtained only in the pressure canner (240°F–250°F). More than 6 hours is needed to kill the spores at boiling temperature (212°F). The toxin is destroyed by heating to 176°F or boiling for 10 minutes to 20 minutes.
Honey may contain spores from the bacterium Clostridium botulinum. These spores can cause infant botulism, a type of severe food poisoning that produces toxins that affect nerve function and can lead to skeletal muscle paralysis.
Symptoms of Infant botulism
Most infants with infant botulism will initially develop constipation, poor feeding, loss of head control, and difficulty swallowing, which can progress to difficulty breathing and respiratory arrest.
Incidence of botulism is low, but the mortality rate is high if not treated immediately. The disease can be fatal in 5 to 10 percent of cases.
Approximately 100 cases of infantile botulism are reported annually in the United States. Around 20% are linked to raw honey consumption, with most cases occurring in infants from immigrant families. In the majority of affected infants, the source of C. botulinum spores remains unidentified.
A stool or enema specimen is required for definitive diagnosis of infant botulism. Enemas should be performed with sterile, non-bacteriostatic water. Diagnostic testing is done through your state public health department laboratory. CDC provides testing services for some state public health departments.
Infant botulism is a true pediatric emergency and prompt recognition and treatment is imperative in order to decrease mortality. While mortality was approximately 90% one hundred years ago, today infant botulism carries a much better prognosis due to the advent of antitoxin treatment with a mortality closer to 15%.
Infant botulism is by ingested C botulinum spores that germinate in the infant's intestine. Sources include environmental spores from soil, dust, or contaminated food products such as honey and corn syrup. Despite the association with honey, most cases occur without known exposure to it.
The incubation period is variable, but neurological symptoms of foodborne botulism usually appear within 12-48 hours with a range of 6 hours to 8 days after eating contaminated food.
Infant botulism is highly treatable, and the vast majority of babies recover. Treatment involves an antitoxin, Botulinum Immune Globulin, known by the brand name of BabyBIG®, which has been the first-line treatment for infant botulism since 2003.
The time course of serious complications from the onset of symptoms often ranges from less than 24 hours to greater than 1 week [12]. Our patient did not experience any of these complications.
