Dihydrocodeine and tramadol are the most direct equivalents to codeine in terms of potency and use as weak opioids for moderate pain. Other non-opioid options are often more effective for acute pain management.

For example, since both codeine and heroin have morphine as a metabolite, mistaken accusations of morphine or heroin abuse may arise in patients prescribed codeine.
Adults Codeine phosphate may be given orally in doses of 15 mg – 60 mg every 4 – 6 hours as needed. If these doses fail to relieve pain, larger doses rarely succeed and may give rise to restlessness and excitement. The maximum daily dose in adults should not exceed 300 mg.
The usual dose of codeine is 15mg to 60mg. The usual dose for treating pain is: adults – one or two 30mg tablets, taken every 4 hours, up to a maximum of 8 tablets (240mg) in 24 hours. children (aged 12 to 17 years) – one or two 30mg tablets (or one or two 5ml spoonfuls of liquid), taken every 6 hours.

Some people can become more sensitive to pain (hyperalgesia). If this happens, your doctor will reduce your dose gradually to help these symptoms. It's possible to become addicted to codeine. For this reason, your dose will be reviewed to make sure you're only having the amount you need to control your pain.
The limited conversion to morphine and fewer side effects of codeine has made it popular for infants and young children, particularly when a single dose is involved. There is some evidence that codeine is associated with less nausea and vomiting than morphine.
Both tramadol and oxycodone are effective drugs for treating pain. However, because oxycodone is so muchmore potent than tramadol, it is more effective for more severe pain. Tramadol is generally used for less severe pain than oxycodone for this reason.
Morphine
An empty stomach can also accelerate the effect; the active ingredient doesn't have to fight its way through any chewed food first. Even so, it's not recommended to take tablets on a completely empty stomach, as many active ingredients can attack the stomach lining.
