Work/life balance was voted the most important career aspect by 29% of physicians in our survey. Because they typically work a fixed number of office hours, dermatologists develop better long-term relationships with their patients and with their external friends and family.
Dermatologists have the highest job satisfaction among 29 medical specialties, while internal medicine physicians have the lowest, according to a 2022 speciality report from Medscape.
Of note, allergy and immunology physicians also spend the least amount of time on paperwork and administrative tasks out of any specialty on our list at just 14.6 hours per week. For perspective, this is a similar amount of time to dermatology which is known for being one of the best lifestyle specialties.
1 | Family Medicine
The number one least competitive specialty is family medicine with a total of 10 points. This specialty has held this spot for quite a few years now. Family medicine is the center of primary care.
As much as 10% of DO graduates do not match with a residency program. Doctors of osteopathy had lower match rates than students from medical doctor programs.
In 2021, 28.0% of U.S. MD graduates who chose otolaryngology as their only choice of specialty did not match, higher than orthopedic surgery (21.7%) and plastic surgery (20.1%), according to the National Resident Matching Program® (NRMP).

Surgical specialities tend to yield the highest pay, while pediatricians typically take home the least, according to a recent report from Doximity. The networking service for medical professionals has released its 2023 Physician Compensation Report, tracking trends in physician pay nationwide.
Physicians' happiness varies greatly by specialty, with plastic surgeons reporting the most contentment outside of work, a Feb. 24 Medscape report found. The report is based on survey responses from 9,175 U.S. physicians polled between June 28 and Oct. 3, 2022.
On the most fortunate side, nearly half of anesthesiologists (48.3%) and radiologists (48.8%) who answered our survey took more than 4 weeks of vacation each year. About a third to a half of physicians get in 2-4 weeks of vacation time a year.

1. What are the highest paid residencies in the US? Allergy & immunology, hematology, medical geneticists, rheumatology, and most forms of specialized surgery top the list. However, it's important to remember that these are subspecialty residencies, aka fellowships, and so are effectively PGY4+ residencies.
As expected, the surgical sub-specialties work the longest hours with neurosurgery leading the pack and general surgery following closely in second. Of the medical specialties, suprisingly, neurology works the most hours, with pediatrics and internal medicine following close behind.
With 6-7 years of total residency required, neurological surgery residencies are among the longest residencies. The complexity and sensitivity of neurological surgery demands extensive training for mastery, and the amount and structure of PGY varies between residency programs.
