Typically people are diagnosed with multiple sclerosis (MS) between the ages of 20 and 40, but late-onset MS (LOMS) affects people age 50 and older. LOMS usually progresses faster than MS diagnosed at a younger age.
Here's where MS (typically) starts
Although a number of MS symptoms can appear early on, two stand out as occurring more often than others: Optic neuritis, or inflammation of the optic nerve, is usually the most common, Shoemaker says. You may experience eye pain, blurred vision and headache.
We know early treatment improves long-term health and wellbeing by slowing down the build up of irreversible damage and reducing the number of relapses people experience. Starting MS treatment early is best but if you start later it can also have some benefits.
MRI scan. An MRI scan is a painless scan that uses strong magnetic fields and radio waves to produce detailed images of the inside of the body. It can show whether there's any damage or scarring of the myelin sheath (the layer surrounding your nerves) in your brain and spinal cord.
But if you have benign MS, you might have some of the following symptoms, even though you can walk and most of your body still works well: Tiredness. Muscle spasms. Vision problems.
While there is no definitive blood test for MS, blood tests can rule out other conditions that cause symptoms similar to those of MS, including lupus erythematosis, Sjogren's, vitamin and mineral deficiencies, some infections, and rare hereditary diseases.
MRI scans are an important way to help health care providers figure out if a person has MS or not, but MRI scans cannot diagnose MS by themselves. While it is true that almost all people with MS will have lesions on MRI, not all people with MRI lesions have MS.
Symptoms can appear suddenly and then vanish for years after the first episode, or in some cases never reappear. The symptoms of MS vary greatly and can range from mild to severe. Most people suffer minor effects.
MS itself is rarely fatal, but complications may arise from severe MS, such as chest or bladder infections, or swallowing difficulties. The average life expectancy for people with MS is around 5 to 10 years lower than average, and this gap appears to be getting smaller all the time.
The study found that people with MS lived to be 75.9 years old, on average, compared to 83.4 years old for those without. That 7.5-year difference is similar to what other researchers have found recently.
Sjogren's syndrome is an autoimmune disease that can mimic some of the symptoms of MS such as fatigue and joint pain.
MS is best detected by a neurological examination and painless imaging studies of the brain and spinal cord using magnetic resonance testing (MRI). An ophthalmologist also can use a test called an optical coherence tomography (OCT) to determine if the optic nerve has been affected by MS.
People should consider the diagnosis of MS if they have one or more of these symptoms: vision loss in one or both eyes. acute paralysis in the legs or along one side of the body. acute numbness and tingling in a limb.
Signs and Symptoms
Characteristic lesions are located in the periventricular and juxtacortical regions, in addition to the brainstem, cerebellum, spinal cord, and optic nerve.
Diagnosis and early intervention
As optic neuritis is the presenting sign of MS in up to 30 percent of patients, the eye exam can lead to the initial systemic diagnosis.
Cluster headaches have been linked to MS lesions in the brainstem, especially in the part where the trigeminal nerve originates. 7 This is the nerve involved with trigeminal neuralgia—one of the most painful MS symptoms.
That said, certain symptoms should prompt you to seek help. If you're experiencing blurred vision, numbness, weakness or dizziness at the same time and for more than a day, those symptoms could be potential early signs of MS. It's important not to let too much time slip by before seeing a doctor.
Although MRI is a very useful diagnostic tool, a normal MRI of the brain does not rule out the possibility of MS. About 5 percent of people who are confirmed to have MS do not initially have brain lesions evidenced by MRI.
One of the first questions many people have when they're diagnosed with MS is: “Will I still be able to drive?” The good news is that most people with MS continue to drive as normal.
A wide range of conditions can be mistaken for MS, including: migraine, cerebral small vessel disease, fibromyalgia, functional neurological disorders, and neuromyelitis optica spectrum disorders, along with uncommon inflammatory, infectious and metabolic conditions (1, 3).
