Your actual SIM also has a series of numbers printed on it. Your PUK code is the 8 digit number found on either the SIM or the additional SIM packaging. By default, your PIN code is the last 4 digits of your PUK code – but obviously if you've changed your PIN code yourself, this will no longer apply.
When you purchase a SIM card, it comes in a little package, and the PUK code should also be included in it. If you still have the package, look for the plastic card that the SIM was snapped out of. The 8 digit PUK code should be printed on the back, together with the SIM PIN code.
The PUK (Personal Unblocking Key) code is a code consisting of 8 digits. It is used to unblock your SIM card when you entered 3 times a wrong PIN code. A card blocked by the PUK code cannot be unblocked; it cannot be used any further and you have to replace it.
Your SIM PIN is different to your PUK and your phone passcode. It's a 4-digit number that lets you lock your SIM card to one device. By default, your SIM PIN is the last four digits of your PUK. You can change your SIM PIN through your device settings.
Unfortunately, there is no option for you to manually unlock your phone if forgot your PUK Code. The only option left is to call the customer support of your internet service provider to request unlocking your SIM Card.
Call your network provider.
The PUK code is unique to your SIM card, so you can only get this from your mobile network. Sometimes the network will give you this when you first get the SIM card, but not all networks do this.
A PUK is required when you want to unlock a phone. You'll need one in the event that your handset is locked by your current network and you want to use a SIM card from another network. You'll also need to get a PUK code in the event that you mistakenly enter the wrong PIN code and you are automatically locked out.
Default PIN means the four digit pin code issued to you upon registration to be used once for the purpose of activating your Account and which for security purpose you must change to a PIN immediately thereafter.
You can remove the SIM lock by performing an unlock procedure on the phone. Although the specific unlock instructions differ according to your phone model, the procedure consists of obtaining an unlock code from your existing mobile provider. You can also buy a code online if the provider is unable to give it to you.
A PUK code is a security feature on your sim card that, where you can choose to have your sim request a PIN code when your phone is switched on. If this code is entered incorrectly three times, your sim card will lock itself, and this can only be unlocked using your PUK code.
Don't get confused between a PUK code and a PAC code. A PUK code unblocks a phone when an PIN has been entered incorrectly too many times. A PAC code is obtained when someone wants to leave a service provider and use their device on another network.

If it started recently, then it is most likely due to a recently installed app or an app update. For a start, delete app cache from settings storage, reboot to recovery and wipe system cache. See if that helps.
If your SIM card is permanently blocked as a result of entering the wrong PUK repeatedly ten times, the only way out is to contact your service provider. Perhaps, they'll need to help you retrieve your line (by linking your phone number to a new SIM card).
You can sometimes find this on your carrier's website after you log in, and you can usually also obtain it by calling customer service or taking the phone to a carrier store. Once you have the PUK code, you can enter it and reset your SIM PIN to one you know.

If you forgot or don't know your SIM PIN
Contact the carrier that gave you the SIM card. If you aren't sure which carrier to call, remove your iPhone SIM card or iPad SIM card and check the card for the carrier's name or logo. Ask your carrier to help you unlock your SIM card using the default SIM PIN or PUK code.
(Subscriber Identity Module lock) A function in GSM cellphones that restricts the phone's use to a particular cellular carrier identified by a SIM card. SIM locks are used to ensure subscribers finish their service contracts. If the contract period is over, the phone may be unlocked by entering a code from the carrier.
Contact your mobile service provider and say you want to unlock your phone. Your service provider will review your account to confirm you meet all requirements to have your phone unlocked for free. You may not be eligible to unlock your phone if you owe money or have not fulfilled the service contract.
The best way to quickly check whether a mobile phone is locked or not is to slip the SIM card out, and put in one from another network. If it can make calls, send texts, and connect to the internet, that means your phone is unlocked. Just make sure that the two SIMs don't actually connect to the same network.
