Cochlear sclerosis (or cochlear otosclerosis) is a form of otosclerosis where abnormal bone growth in the inner ear (otic capsule) affects the cochlea, leading to sensorineural or mixed hearing loss, not just conductive loss from the common stapes issue. It's characterized by bone demineralization and hardening within the cochlea, often visible on CT scans as a "double ring sign," causing progressive hearing impairment, tinnitus, and sometimes vertigo.
Otosclerosis (oh-tuh-skli-ROH-sis) is a condition that causes hearing loss. The term “oto” means “of the ear” and “sclerosis” means “abnormal hardening of body tissue.”
Once you develop tympanosclerosis, the scar tissue doesn't just go away. But unless you have hearing loss, you probably don't need to do anything at all. However, people with hearing loss due to tympanosclerosis usually need treatment.

Many people with otosclerosis first notice that they are unable to hear low-pitched sounds or can't hear a whisper. Some people may also experience dizziness, balance problems, or tinnitus. It is also important to pay attention to signs of hearing loss and to get your hearing tested. Signs include: Speech and other sounds seem muffled. Trouble hearing high-pitched sounds.
Stem Cell and Gene Therapy (Future Potential)
Research on stem cell and gene therapy suggests that regenerating cochlear hair cells or auditory nerve fibers may one day restore hearing. Gene therapy trials are investigating how to reprogram damaged cells. Stem cell studies show 40-60% regrowth in animal models.
Although hearing aids and cochlear implants can ameliorate the symptoms somewhat, there are no known treatments to restore hearing, because auditory hair cells in mammals, unlike those in birds or fish, do not regenerate once lost.
Tympanosclerosis may not always present with obvious symptoms, and many people may not experience any significant discomfort. However, when symptoms do occur, they can include: Ear Discomfort: This can range from mild to moderate pain or a feeling of fullness in the ear.
The primary indicator of an ear stroke is a rapid loss of hearing in one ear, but other symptoms often accompany it. These may include a feeling of fullness in the ear, dizziness, vertigo and tinnitus, which is a ringing or buzzing sound. Some people might also feel off-balance or experience nausea.
The veteran's tympanosclerosis of the left ear is evaluated as noncompensable under Diagnostic Code 6211 of the VA Schedule for Rating Disabilities. The noncompensable evaluation is the only evaluation under this code and contemplates perforation of a tympanic membrane.
While otosclerosis can lead to severe hearing loss, it rarely results in total deafness. People with otosclerosis often are unaware that they have the disorder until they experience hearing loss, which gradually worsens.

A new drug, the FX 322, is said to reverse hearing loss
Research is currently being undertaken by Frequency Therapeutics*, a new gel-based drug that is injected into your ear and designed to promote stem cell growth. This will regenerate and develop hair-like stereocilia cells, which are said to reverse hearing loss.
Those devices cannot help a small population of individuals who do not have a cochlear, or hearing, nerve — these people are unable to perceive sound, no matter how loud, outside of feeling vibration.
