After a close-encounter explosion from a Thunder Spear set off by Zeke Yeager, Levi now has several scars across his face including one across his right eye and is missing both the index and middle fingers on his right hand.
His injuries range drastically, beginning with a massive scar across his right eye which runs along the length of his face. He also loses two fingers on his right hand from the explosion. The scar across his right eye is eventually, and unfortunately, revealed to have blinded him in that eye.

Levi's ending and its meaning
Humanity's Strongest Soldier ended his life implicitly unable to continuously walk on his own two feet. It's likely Levi needed assistance in other areas due to his injuries as well. His entire life had been dedicated to fighting, and he couldn't even walk anymore.
Petra was the member of Levi Squad whose death hurt Levi the most. She was in love with him and he found out that she wanted to marry him after her death. Always putting his safety above anything else, she was the most loyal member of the squad. The Female Titan kicked her into a tree, squishing her to death.
That doubt was put to rest when Nico kissed Levi in an elevator at work and he liked it, essentially confirming that Levi was into dudes. But when Nico found out that he was the first man that Levi had kissed, he got scared off, saying he can't “teach” Levi how to be gay.
While there are many characters that he respects, Erwin Smith is perhaps the only character that Captain Levi has truly loved, which puts Erwin at the very top of the list. Levi's loyalty and devotion to Erwin also indicate that the two were meant to be together.
As was confirmed by Hajime Isayama himself, Levi Ackerman never had a love interest in the series. Namely, Levi Ackerman was created as a character who was simply above falling in love with someone, so Isayama was actually consistent in writing Levi throughout the series.
