Lucas has said “Darth” is a variation of “dark”, though he has also explained the inspiration behind the name were the words “death” and “invader”. There's no actual meaning to the word “darth” in the real world, only the in-universe explanation of a Sith title.
Darth was a Sith title given as both a name and title to the Sith Lords, an order of Force-sensitives who channeled their power through the dark side of the Force. The title, which roughly translated to "Dark Lord," was part of a Sith Lord's mantle, preceding a moniker that replaced the Sith Lord's original name.
Many Sith Lords chose to add "Darth" to their name, so much so that the word is considered synonymous with the dark side of the Force in some circles. It was also taken to signify giving up one's old life. Such examples are Anakin Skywalker becoming Darth Vader, or Jacen Solo becoming Darth Caedus.

While the theory seems quite plausible, Obi-Wan actually calls Anakin "Darth" in A New Hope because creator George Lucas hadn't yet established some elements of Star Wars lore. As the fictional universe expanded into the prequel trilogy, "Darth" became a Sith title as opposed to a first name.

Turns out — as many have suspected — this was a low-key revelation that Palps is Anakin's daddy. And the new comic book proves it. The new-ish comic book issue, Darth Vader #25, features a flashback scene in which Anakin is able to relive certain parts of his life, including his birth.

Even though Kylo Ren uses a red lightsaber, harnesses the dark side of the Force, and wears a cape, he is not a Sith. Because of that, he can never hold the title of Darth. Instead, he was just a chaotic mess of the dark side (until he eventually found his way back to the light).
In Star Wars lore, a Sith Lord cannot simply name themselves. Instead, their Master must give their apprentice a name. For this method, ask a friend, acquaintance, family member, or sworn enemy to become your Master and come up with your Sith Lord name!
Obi-Wan gave the lightsaber to Anakin's son Luke, who used it in taking his first steps along the Jedi path.
