End-stage heartworm disease (Class III/IV) involves severe symptoms like labored breathing, fainting, extreme exercise intolerance, a swollen abdomen (ascites), pale gums, and dark/bloody urine (Caval Syndrome), indicating advanced heart and lung damage, liver issues, and potential organ failure, with sudden death being a risk.
Stage 4: Severe Disease and Life-Threatening Complications
The most serious stage of heartworm disease can be life-threatening. In severe cases, worms can block blood flow, leading to a sudden emergency known as caval syndrome. Immediate surgical intervention is often required to save the pet's life.
In more advanced stages of heartworm infections, your dog will find it hard to complete normal physical tasks like eating. If you notice weight loss and a lack appetite in your dog, then you should take him to the vet immediately to rule out heartworms and other illnesses. Shallow and rapid breathing.
Signs of Heartworm Disease in Dogs
How long can a dog live with heartworms? Dogs are able to live for years with this parasite, however, their condition will continuously worsen. It's a dangerous disease overall, which is why some people have even described untreated heartworms as the “slow kill.”
Heartworm is not necessarily painful for dogs, but it is uncomfortable. This discomfort is primarily due to the inflammation it causes in the heart and the lungs.
As they break up, they are carried to the lungs, where they lodge in the small blood vessels and are eventually reabsorbed by the body. This resorption can take several weeks to months, and most post-treatment complications are caused by these fragments of dead heartworms.

The cough associated with heartworm disease in dogs is often likened to the sound of a goose honking. If your dog tends to get into coughing fits after activity or eating, and if those coughs sound harsh and honk-like, then she may have heartworm disease or another heart related concern.
Apples: Apples are great for dogs with heartworms due to their high levels of vitamin A, vitamin C and fiber. Apples in your dog's diet may help repair damaged tissue and blood vessels and support the health of their vital organs.
Another clear indicator of heartworm disease is lethargy. If your dog normally has a lot of energy but is now spending a lot more time sleeping or avoiding interacting with you, this may be due to their heart having to work harder to pump blood.
Heartworm disease can also lead to liver or kidney failure. Dogs that are exposed to a large number of infective larvae at once are at great risk of sudden death due to massive numbers of developing larvae bombarding the vascular system.
Also bring a fresh stool sample for them to check for the presence of worms. Finding microscopic intestinal worm eggs in the stool is the most common way to diagnose worms in dogs. Heartworms are diagnosed with a blood test, an X-ray of the chest, or an ultrasound of the heart.
Last Stages of Congestive Heart Failure in Dogs
Difficulty breathing even while resting, frequent bouts of coughing, bluish-grey color gums, fainting when rising to stand and a reluctance to walk are all common signs of the end stages of congestive heart failure in dogs.
Moxidectin-doxycycline (moxi-doxy) provides a viable alternative to no treatment at all, in cases where arsenical treatment is not possible. Based on current evidence, the most effective non-arsenical treatment regimen is doxycycline 10 mg/kg PO q 12 or 24 h for 28 days, combined with topical moxidectin at label dose.
It is a dry cough, but it comes frequently and is usually brought on by activity. In dogs who are in the first stages of heartworm disease, the cough may be brought about by even a small amount of play or exercise.
Smoke, dust, pollen, or strong cleaning products may trigger coughing fits. Dogs may also react to perfumes or aerosol sprays. If you notice your dog coughing after you've cleaned or sprayed something indoors, consider removing those irritants from your home environment.
Coughing or Asthma-like Attacks: Respiratory issues are a common sign of heartworm disease in cats, often mistaken for feline asthma.
Early Symptoms of Lungworm in Dogs
Stage 4: Dogs in stage four of heartworm disease have very visible heartworm disease symptoms. These symptoms are accompanied by long-term implications for the dog's health. These dogs are very ill. The symptoms are similar to Stage 3 but more severe.
Anecdotal evidence suggests that herbs such as garlic, black walnut and wormwood, and the homeopathic heartworm nosode, may actually prevent as well as treat infection. Unfortunately, I have not yet been able to find substantive proof that these therapies can reliably and safely prevent or treat infection or disease.
Recognizing Symptoms Quickly
Heartworm disease can progress slowly, sometimes taking one to two years to reach advanced stages. The initial larvae reach adulthood in approximately seven months, and then they multiply. The length of time makes recognizing symptoms difficult because the signs may seem gradual.
It takes several months for the body to absorb the decomposed heartworms. Most problems after treatment occur due to these decomposing fragments. To reduce the risk, your dog should avoid exercise and be kept calm for the first month after treatment.
" Heartworm preventives do not kill adult heartworms. Giving a heartworm preventive to a dog infected with adult heartworms may be harmful or deadly. If microfilariae are in the dog's bloodstream, the preventive may cause the microfilariae to suddenly die, triggering a shock-like reaction and possibly death.
No one wants to hear that their dog has heartworm, but the good news is that most infected dogs can be successfully treated. The goal is to first stabilize your dog if he is showing signs of disease, then kill all adult and immature worms while keeping the side effects of treatment to a minimum.
