Holmes expressed multiple symptoms of the Savant Syndrome. He plausibly could have had other neurological disorders such as Asperger's Syndrome or Bipolar Disorder as well. Savant Syndrome is often paired with other neurological disorders (Bôkkon 69).
If you take a look at Doyle's original character, he is most definitely a primary psychopath – not a “high-functioning sociopath.” He shows no emotion or empathy, and he doesn't respond to stress.
He most certainly has a conscience (and as Hare says, if nothing else, the “hallmark [of a sociopath] is a stunning lack of conscience”). In other words, Holmes has emotions—and attachments—like the rest of us. What he's better at is controlling them—and only letting them show under very specific circumstances.
