In most cases, however, an epidural will not be given until the mother is at least 3-4 centimeters dilated. Once the mother is fully dilated most doctors and hospitals will consider it too late for an epidural to be given.
Recent research has suggested that 6 cm of cervical dilation should be the threshold for the active labor phase, and it has confirmed that epidural analgesia (EA) is a safe method of pain relief during labor.
A woman can get an epidural at almost any time in labor if she can remain relatively still; however, an epidural is generally not given if the baby is close to being delivered. Some women have heard they need to be dilated (meaning the cervix opens in preparation for birth) a certain amount before an epidural is given.
Active labor
This is when you head to the hospital! Active labor usually lasts about 4 to 8 hours. It starts when your contractions are regular and your cervix has dilated to 6 centimeters.
Although it is the shortest phase, the transition phase is the most challenging. Transition typically lasts 30 minutes to 2 hours as your cervix fully dilates from 8 cm to 10 cm.
Doctors have to wait until the cervix is at least 4 centimeters dilated before doing an epidural. Otherwise, the epidural will slow the process down too much. However, once the cervix becomes fully dilated it is too late for an epidural to be given.

Getting up and moving around may help speed dilation by increasing blood flow. Walking around the room, doing simple movements in bed or chair, or even changing positions may encourage dilation. This is because the weight of the baby applies pressure to the cervix.
Once the cervix has reached 10 cm, it is time to push the baby out. Contractions continue but also produce a strong urge to push. This urge might feel like an intense need to have a bowel movement. This stage can last anywhere from a few minutes to a few hours.
If you are bleeding heavily or are suffering from shock, you will not be given an epidural for safety reasons. 2 Since many women tend to have lower blood pressure with an epidural, this may be made even more dangerous with the lowered blood pressure of some of these problems.
During this stage, your cervix gradually dilates to about 6 cm. This is the longest part of labor and can take anywhere from a few hours to a few days, although between 8 to 12 hours is common.
A fully dilated cervix is 10 centimeters open.
This is about the size of a bagel, and means that when your cervix is measured with two fingers, they can be stretched 10 centimeters across.
In some women, the foetal head is not in the right position and doesn t apply aptly to the cervix, which is why the cervix doesn t open. This can lead to difficulty in dilation.
Warning Signs of Premature Labor
Menstrual-like cramps felt in the lower abdomen that may come and go or be constant. Low dull backache felt below the waistline that may come and go or be constant. Pelvic pressure that feels like your baby is pushing down. This pressure comes and goes.
You may be admitted to the hospital if your cervix is dilated past six centimeters, your water is broken, or you have special circumstances (as determined by your healthcare provider or midwife).
It is possible! I have been with many women in labour through the night and helped them fall asleep. They have continued on to have positive, healthy births and feel super proud of their efforts, albeit ready for a good sleep afterwards!
Many pregnant people go into labor within 12 hours of their water breaking. However, 1 in 10 will not go into labor on their own after a day or so. A person may consider contacting their doctor if they do not experience any signs of labor within several hours of their water breaking.
