With Stage 0 Leukemia (Chronic Lymphocytic Leukemia or CLL), you can often live for many years, even decades, with some patients experiencing a normal lifespan, as this early stage often requires no immediate treatment, but survival varies greatly depending on individual factors like genetics, age, and specific disease markers, with statistics showing long-term survival rates over 10 years.
The 10-year survival rate for a score of 0 to 1 is about 86%. As the score gets higher, the survival rate gets lower. For chronic lymphocytic leukemia with a score of 7 to 10, the chance of surviving at least five years is about 23%. The 10-year survival rate for a score of 7 to 10 is 0%.
Stage II: This stage is characterized by an enlarged spleen or liver in addition to swollen lymph nodes. Stage III: Indicates anemia due to leukemia cells crowding the bone marrow. Stage IV: The last stages of leukemia in CLL involve low platelet counts, which increase the risk of bleeding.
Symptoms of chronic lymphocytic leukemia may not appear for many years. However, if symptoms do occur, they can include swollen lymph nodes, extreme fatigue, and easy bruising. Treatment may not be necessary but could include chemotherapy, targeted therapies, and immunotherapy.
The Rai system divides CLL into 5 stages based on the results of blood tests and a physical exam: Rai stage 0: Lymphocytosis; no enlargement of the lymph nodes, spleen, or liver; red blood cell and platelet counts are near normal.
Follow-Up Care
After being treated for Stage 0 melanoma, you should conduct regular self-exams of your skin and lymph nodes and have an annual, full-body skin exam performed by a trained dermatologist for the rest of your life. Imaging tests may be ordered as needed to monitor for recurrence.
Stage 0 breast cancer is considered a noninvasive type of breast cancer. A preinvasive, or noninvasive, cancer type is one that has not spread into nearby breast tissue. This means the cancer cells have not metastasized, or spread, into other areas of the breast or body.
When detected and treated early, acute leukemia is curable in many cases. "Even when there are a lot of factors, such as advanced age and other medical conditions, we estimate that half of acute leukemia cases can be cured, which is high for such an aggressive cancer," says Dr. Shah.
CLL and SLL are slow-growing (low-grade or 'indolent') cancers. Many doctors group them together as a type of non-Hodgkin lymphoma. They usually behave like a long-term (chronic) condition that needs treatment from time-to-time to keep it under control. CLL and SLL are treated in the same way.
Some people with leukemia or myelodysplastic syndromes have bone or joint pain. This bone pain is most often felt in the long bones of the arms and legs, in the ribs, and in the breastbone.
For some, living 20 years with leukemia is a real possibility. This leukemia survival rate is more likely for those diagnosed young or with less aggressive types like chronic lymphocytic leukemia (CLL). The 5-year survival rate for all types in the United States is 67%. This shows a hopeful outlook for many patients.
Night sweats, which can be a symptom of leukemia itself or treatments, might also disrupt sleep. It's very common to experience fatigue during all stages of CLL.
How leukemia forms. In general, leukemia is thought to occur when some blood cells acquire changes (mutations) in their genetic material or DNA. A cell's DNA contains the instructions that tell a cell what to do. Normally, the DNA tells the cell to grow at a set rate and to die at a set time.
Stage 0 cancer is the earliest and most localized form of cancer. It is noninvasive, meaning it hasn't spread to nearby tissues. Because of this, stage 0 cancers are highly treatable and often curable, with survival rates approaching 100% in many cases.
However, these statistics vary greatly according to the specific subtype of disease:
Stage 0 – A patient has high levels of white blood cells, but no other physical symptoms. Stage 1 – A patient has high levels of white blood cells and enlarged lymph nodes. Stage 2 – A patient has high levels of white blood cells and is anemic. He or she may also have enlarged lymph nodes.
Stage 0 is low risk. People in stage 0 may have no symptoms of leukemia. Lymphocytosis is usually detected as an incidental finding (something doctors discover without looking for it) on routine blood tests. Symptoms, if there are any, usually don't interfere with daily life.
Chronic lymphocytic leukaemia is a rare type of cancer that affects the blood and bone marrow. It usually develops very slowly and does not always need to be treated straight away.
At the time of diagnosis, patients can have very, very high white blood cell counts. Typically a healthy person has a white blood cell count of about 4,000-11,000. Patients with acute or even chronic leukemia may come in with a white blood cell count up into the 100,000-400,000 range.
Even with normal blood counts, many leukemia patients require ongoing maintenance therapy to stay in remission. In some cases, patients in remission undergo a stem cell transplant to maintain remission. Patients who remain in continued complete remission for an extended period of time are considered cured.
One of the earliest signs of leukemia is often persistent fatigue and weakness, stemming from anemia, alongside frequent infections, easy bruising, or unusual bleeding (like nosebleeds/gum bleeding) due to issues with blood cell production. Other common early indicators include fever, night sweats, unexplained weight loss, and swollen lymph nodes.
So, is leukemia a hereditary disease? In most cases, the answer is no. Leukemia is largely caused by genetic mutations acquired during a person's lifetime. However, specific gene mutations can increase the likelihood of developing leukemia within families.
It's important to keep in mind that precancerous lesions can be found almost anywhere in the body and that early detection is one of the most important parts of curing cancer. However, in some instances, doctors can detect a cancer so early that it is referred to as a “precancerous” lesion or a “Stage 0” cancer.
Others caution that, while not invasive yet, some stage 0 cancers can become dangerous over time, especially if left untreated. Research suggests that up to 40% of untreated DCIS cases may eventually develop into invasive breast cancer, although the risk in any given year is relatively low at around 3%.
Ductal carcinoma in situ (DCIS)
This type of cancer develops in the milk ducts, makes up roughly 1 in 5 new breast cancer cases each year and is, by far, the most common of the two forms of stage 0 breast cancer. If it isn't treated, DCIS may become invasive. In fact, it does in up to 50 percent of all cases.
