The Overwatch comic "Legacy" explains that Ana had the opportunity to take out Widowmaker during a mission, but hesitated when she recognized the person she was aiming at. This mistake cost her dearly, as Widowmaker then shot Ana in her eye and left her mortally wounded.
Ana wasn't “young Ana” when Widowmaker shot her eye out - Ana already had grey hair. She was ~54, and Widowmaker was ~27.

A technical marvel and loyal combatant, Orisa stands ready to protect and fight for her home of Numbani. Being a new robotic construction during the events of Overwatch (2076), Orisa technically remains the "youngest" hero in the game.
Overall Ana is very strong, though Kiriko +Hog is very meta and is diminishing the value she can get with her antinade. If you aren't getting too harassed (or your team is doing a good job protecting you) and you're able to get good value out of your cooldowns, she's still great.
A founding member of Overwatch and once renowned as the greatest sniper in the world, Ana Amari comes from a long line of decorated military veterans. Though she was thought to have perished in a firefight with Talon, Ana has rejoined the fray to protect her country, family, and closest allies.
Ana's Biotic Rifle becomes a hit-scan weapon as long as she is aimed down her sights. This makes Pharah a vulnerable target to skilled Ana's that are able to take her down if she doesn't receive healing. Three shots from Ana's Biotic Rifle are more than enough to eliminate a Pharah out of the sky.
