He is informed by Doctor Holford (Aneurin Barnard) that he has tuberculoma and has at best, a year and a half to live. He is certain now that his death is imminent.
He had tuberculoma in his brain stem, a growth caused by the same bacteria as the lung disease, doubtless picked up from daughter Ruby (Orla McDonagh). It wasn't infectious but it was inoperable. The symptoms (seizures, dissociation, hallucinations, sharp cheekbones – I may have added the last one) chimed with Tommy's.
PSA: Thomas Shelby got the clap from a French girl and I am still not over it. Even better is that this happened enough that in WWI there was a song about English soldiers doing that.
Alfie revealed that he was suffering from skin cancer, which he attributed to the poisonous gas used in the Great War; Alfie had intended for Tommy to discover his betrayal, track him down, and kill him so Alfie could die on his own terms, rather than the Mafia.
Duke isn't the only surprise Tommy receives in 'Sapphire'. Following the X-Rays he was forced to take at the sanatorium where Ruby was treated and died, Tommy also gets a diagnosis. His lungs are clear of tuberculosis, but he has developed a tuberculoma.
However, it turns out that Alfie had an ulterior motive with his betrayal. He had just found out that he had cancer, and he knew Tommy would kill him before the cancer did.
That whisper is the semi-public admission that Tommy is shaking his blood-stained hands with Duke, and expecting him to carry on his dark deeds.
After a drunken Finn let slip that the gang was going to shoot a fascist on the night of Mosley's Bingley Hall rally, Grade phoned Lorna McKee, and the IRA stopped the assassination by killing sniper Barney Thompson, Aberama Gold and Polly Gray.
Over five or so years, they became close, drinking whiskey, snorting cocaine and womanising together. Finn didn't realise that Billy was also working as an informant, telling Peaky Blinders secrets to the IRA.

Sabini introduces himself to Thomas Shelby by having his men beat him to within an inch of his life, then Sabini opens Tommy's mouth and rips out one of his teeth and cuts his cheek.
Cillian Murphy, who plays Tommy, was asked about this very topic during an interview with BBC Radio One. The actor explained: "That comes from the fact that the cigarettes, we cut the filter off or do we leave the filter on I cannot recall, but anyway if you don't rub them across your lips initially, they will stick.
Tommy whispered something in Duke's ear before he turned his back on his family for what he thought was forever (Tommy's diagnosis of terminal tuberculoma was found to be fake) but exactly what he said was never revealed.
The whiskey that is drunk in Peaky Blinders is the Gallglennie Whiskey. However, this whiskey does not really exist but it was invented by the makers of the series. By the way, the actors do not drink real whiskey during the filming. Instead of whiskey they use tea.
If you've ever noticed that Shelby never eats on the show, that's because it was an accidental choice that later turned into an intentional decision. Once fans noticed this quirk, Murphy and Knight decided to turn it into a “Tommy thing,” which added to the character's obsessive industriousness.
After seeing her, Tommy finds a newspaper announcing the wedding of Lady Diana Mitford and Sir Oswald Mosley and sees Dr Holford pictured in attendance. He soon realises that his doctor has deceived him and that he isn't suffering from tuberculoma at all. Read about the real Diana Mitford here.
Despite the plan being a ruse, Tommy is upset with Michael for not warning him about it, since as far as he knew, he was letting Tommy die. Michael says that he did it to protect his mother.
Eventually, Peaky Blinders leader Tommy Shelby (Cillian Murphy) discovered Finn's informant status and gave him false intel that thwarted both the planned murder of Tommy at the hands of cousin Michael Gray (Finn Cole) and the planned murder of older brother Arthur Shelby (Paul Anderson) at the hands of the IRA.

From one brother to the other, Finn Shelby (Harry Kirton) is banished from the family for his role in bringing a traitor in Billy Grade (Emmett J. Scanlan) into the fold. Finn promises vengeance on Duke Shelby (Conrad Khan), Tommy's firstborn son and heir, ordering him banished by order of the Peaky Blinders.
“What he would be diagnosed with nowadays would be paranoid schizophrenia.” This lead to the dramatic ending of the series, which ended on the cliffhanger of Tommy holding a gun to his head before the screen cut to black.
At John's funeral, Polly is the most upset at Thomas for using his own brother's funeral as a trap, and walks away. She meets the hired hitman Aberama Gold for the first time.
Series 5. Alfie is revealed to be alive, living in Margate. His face is visibly scarred from Tommy's shooting, leaving him blind in his left eye. Alfie is acutely aware of the rise of fascist politician Sir Oswald Mosley (Sam Claflin), as well as Tommy's career in politics.
Enter a visibly shaken Arthur, who Alfie greets with an enthusiastic “Arthur, Shalom!” He then apologizes to Arthur for getting him thrown in jail and praises him for “allowing Jesus into his life.” Arthur refrains from smashing Alfie's skull with a crystal ashtray despite the relative ease with which Alfie is able to ...
This information became useful in the finale of that season when Tommy went to kill Alfie because of the bad blood between them. Alfie seemingly accepted his fate, stating that "there's an honorable reason now to pull that trigger" and he even forced Tommy to shoot him in self-defense when Tommy showed hesitation.
