In an early Revenge of the Sith draft, Palpatine confirmed that Darth Plagueis used the Midi-chlorians to start the cell division that created Anakin Skywalker.
No. Anakin's birth was the force's punitive response to Plagueis's attempt to influence midichlorians using the dark side. Plagueis desperately wanted to force the midicholrians to create the ultimate Sith warrior. The Force responded by birthing a child destined to destroy the Sith.
Is Darth Plagueis Anakin's father? No. In legends, Anakin was believed (but not confirmed) to be the result of the Force itself lashing back against the machinations of Darth Plagueis.

Although the tragedy has become more of a meme than a Sith Legend at this point, Plagueis was nonetheless one of the most powerful Sith ever. Plagueis was the master of Darth Sidious and taught the future Emperor everything he knew about the Dark Side.

Usage and history
After Plagueis murdered his master on Bal'demnic in 67 BBY, his lightsaber continued to be in his possession, even at the time he trained Palpatine as his apprentice. When Palpatine executed Pax Teem as retribution for outwitting him and his master he used Plagueis's lightsaber.
The added risk Darth Vader takes in allowing children to live in a situation where he could have killed them proves that he regrets killing Younglings and will avoid harming any more children for the rest of his days, though intense intimidation is a dark strategy to avoid repeating his Youngling massacre.
He wasn't even eligible. In order to be granted the rank of master, a Jedi Knight must first train their own. Padawan to knighthood. When their Padawan became a Knight, they would then be able to attain the rank.

Snoke Is Not a Sith Lord
He was never given one because he isn't technically a Sith Lord. But the fact that he's a bio-engineered being created by Palpatine explains why his training of Kylo Ren followed the Sith mold so closely, since all along Palpatine was trying to manipulate Ren.
