Despite the mutant's enhanced senses, the Hulk somehow sneaks up on him. Wolverine manages to do some damage, though, actually making the Hulk bleed.
10 ONLY TWO THINGS CAN CUT HULK'S SKIN
However, as far as Earth-based weapons, there are two things that have pierced Hulk's skin in Marvel Comics -- Adamantium and Vibranium.

Losing Himself
Not to go too far down the rabbit hole of pop psychology, but what Hulk and Banner are most afraid of is losing themselves to one another. Banner is afraid of what will happen if he Hulks out, his anger takes over, and he can't regain control.
Because of the thickness and strength of the Hulk's skin, it's nearly impossible for any form of artillery to hurt him. Sure, everyone's seen bullets bounce off of the Hulk, but when we say he's bulletproof what we really mean is he's BULLETPROOF.

During the Battle of the Rainbow Bridge, the two take their fight to the water, and Fenris rag dolls Hulk before biting into his leg, making him bleed.
Well, it turns out Bruce has his cousin Jennifer “Jen” Walters to thank for helping to heal his arm back to its full strength following its injury during the final battle with Thanos. Following their car accident, Bruce's blood got into Jen's system which gave her a lethal dose of gamma radiation.
Hulk's inability to break Wolverine and his adamantium claws isn't an indictment of the green monster's strength but more shows how strong Wolverine's metal-coated bones really are. If the Hulk can't snap his claws, then it's unlikely many other heroes or villains could do so, either.

Ultimately, the Merc with the Mouth became a fan-favorite hero, and in modern stories, his healing outshines even that of Wolverine, with Wade able to survive incredible injury, from decapitation to incineration.
The Hulk severely underestimated Wolverine's abilities and thought he could eat him alive. But, killing Wolverine is practically impossible, even if he's devoured into little bits. In this fight, Wolverine was a decisive win against the Hulk, giving him his most disgusting victory against the gamma monster to date.
