As a rule of thumb, a truly hungry baby will rarely choose sleeping over eating. So, if your baby falls asleep in your arms without taking a full feeding, it's likely he was tired — not hungry.
Falling asleep while nursing can be a sign that your baby is latching poorly and/or not getting enough milk due to a decreased supply. These are both important scenarios to rule out to make sure your baby is getting enough nutrition.
Signs Your Baby is Hungry
Head turning to look for the breast. Becoming more alert and active. Sucking on hands or lip smacking. Opening and closing mouth.
Breastfed newborns' longest sleep periods are generally 2–3 hours — this is about how long their small bellies can go between feedings. If newborns do sleep for a while, they'll probably be extra-hungry during the day and may want to nurse more often.
Our resident infant sleep expert, Dr. Natalie Barnett, says yes if your baby is 4-6 months old. “Many, though not all, babies are able to make it through the night without food at 4 months. By 6 months, almost all healthy babies are physically and neurologically able to go 12 hours without food.”
Understanding the Role of Prolactin
Interestingly, prolactin levels have a typical 24-hour cycle — just like the human body's circadian rhythm. Prolactin peaks in the early morning hours around 2-5 a.m., while the lowest prolactin levels happen in the late afternoon to early evening.
The first few days: Your breast milk coming in
Around day three after your baby's birth, your breast milk 'comes in' and your breasts may start to feel noticeably firmer and fuller.
A newborn should be put to the breast at least every 2 to 3 hours and nurse for 10 to 15 minutes on each side. But rather than worry about duration, it's important to know that the best way to ensure that the baby is getting enough breast milk is by feeding frequency, wet and dirty diapers, and weight gain.
Don't worry, baby will ask to be fed as usual as soon as his stomach feels empty again. Your baby may be constipated and appear less hungry than usual, however once this passes everything will go back to normal. Or it may just be that your baby was enjoying the sleep and took longer to wake up.
Lie on your side, with a pillow or two to support your head. A pillow between your knees may help you feel comfortable. Position the baby, lying on his side, so his nose is level with your nipple.
Signs your baby is getting enough milk
They seem calm and relaxed during feeds. Your baby comes off the breast on their own at the end of feeds. Their mouth looks moist after feeds. Your baby appears content and satisfied after most feeds.
Growth spurt
Growth spurts (a period of accelerated growth) are separated by growth plateaus (a period of stagnant growth). During a growth spurt, your baby may appear to be hungrier than usual. If breastfeeding, he may demand nursing more often. If bottle-feeding, he might drain the bottle and want a little more.
On average, most exclusively breastfed babies will feed about every 2 to 4 hours. Some babies may feed as often as every hour at times, often called cluster feeding. Or may have a longer sleep interval of 4 to 5 hours. How often your baby feeds might change depending on the time of day.
Falling asleep at the breast is a normal behaviour and is mostly due to a hormone called cholecystokinin or CCK. CCK makes your baby feel full and sleepy and it is released in your babies gut as soon as they start sucking.
Check how your baby is sucking
Check how he's sucking. If he latches on well and takes long, drawn out pulls, then he's likely hungry and actually eating. But if his sucking motion is shorter and shallower, then he's probably sucking for comfort. You can also check whether he's swallowing the milk.
Is it okay to wake up my baby to eat if I'm getting engorged? You betcha. Don't feel bad about waking baby when you need relief. You have just as much of a right to wake him as he has to wake you.
Older babies may take five to 10 minutes, or less, on each side. Short nursing sessions are normal – and perfectly fine unless your baby is having trouble gaining weight. Some babies are snackers – they nurse for a minute or two, take a break, and then go back.
Newborns should not go more than about 4–5 hours without feeding. Signs that babies are hungry include: moving their heads from side to side. opening their mouths.
Breastfed babies feed often, about every 2–3 hours. Bottle-fed babies tend to feed less often, about every 3–4 hours. Newborns who sleep for longer stretches should be awakened to feed. Wake your baby every 3–4 hours to eat until he or she shows good weight gain, which usually happens within the first couple of weeks.
A baby will unlatch naturally when she's finished breastfeeding. You shouldn't ever have to take your baby off your breast. Whether she falls asleep or just pulls away, she'll know when to unlatch when she's ready.
When he stops suckling and swallowing, or when he falls asleep, you'll want to switch him to the other breast. If he hasn't released the first breast, simply slip your finger into the corner of his mouth to break the suction (and protect your nipple) before removing him from your breast.
Try burping your baby every 2 to 3 ounces (60 to 90 milliliters) if you bottle-feed and each time you switch breasts if you breastfeed. Try burping your baby every ounce during bottle-feeding or every 5 minutes during breastfeeding if your baby: tends to be gassy. spits a lot.

The more milk your baby removes from your breasts, the more milk you will make. Despite views to the contrary, breasts are never truly empty. Milk is actually produced nonstop—before, during, and after feedings—so there's no need to wait between feedings for your breasts to refill.
How do I know whether my breasts are empty? There's no test or way to know for sure. In general, though, if you gently shake your breasts and they feel mostly soft and you don't feel the heaviness of milk sitting in them, you're probably fine.
