In the episode "Birds of a Feather", it is revealed that Melissa is the Black Swan and that the night of the Masquerade Ball she received a note, blackmailing her to wear the costume to the ball. The note also said that she had to distract Jenna, which is why the Black Swan was talking to Jenna.
In the Season 2 finale, Alison's friends learn that Mona has used her talents to manipulate and torment them anonymously. It is also discovered that she has multiple personality disorder, according to her doctor.

Mona's worst Pretty Little Liars actions include being the first A, and while the liars are shocked that Hanna's former best friend would stalk and hurt them, it's also strange that they forgive her later on in the series.

The answer is none other than Mona Vanderwaal (Janel Parrish). Although finding out the original "A" ended up murdering another “A” isn't all that shocking, the events that led to CeCe's death are totally unexpected.
Toby Cavanaugh is a shy person who was blackmailed by his step-sister Jenna to have sexual relations with her, saying that she would tell their parents that Toby had been forcing her and that they would surely have believed her.
Her bad-tempered response causes Spencer to reveal that she knows Melissa is helping Mona. Spencer wonders if someone taunted her sister into something she doesn't want to. Unfolding her arms, Melissa explains that she only helped Mona because she wanted Spencer to see that Alison remains a bully.
Who kidnapped and tortured Hanna? Of all of the questions, this one is probably the easiest to deduce — it's Uber A, because of the character's drive to discover Charlotte's killer. Hanna made a false confession to Uber A that she killed Charlotte, so she was subsequently kidnapped and tortured.
