Alternatives to Colonoscopy for Elderly The alternative to colonoscopy currently considered the preferred alternative to more invasive tests is the stool DNA test. A stool DNA test is performed by collecting a stool sample at home and sending it to a lab for testing.
Dear Reader: The at-home test kit that you received in the mail is known as a fecal immunochemical test, or FIT. As with a colonoscopy, it is a diagnostic tool that looks for signs of colon cancer. The FIT test kit contains the materials you need to collect a small sample of stool and instructions on how to use them.
The FDA approved a tablet alternative, SUTAB (sodium sulfate, magnesium sulfate, and potassium chloride), introduced by Sebela Pharmaceuticals. This tablet option makes colonoscopy prep a much easier process than before.
Preparing for a colonoscopy requires a thorough cleansing of the entire colon before the test, which can lead to dehydration and other problems. And the risk of these possible harms tends to be greater in older people, Dr. Umar added.
Stool tests.
Currently, three types of stool tests are approved by the US Food and Drug Administration (FDA) to screen for colorectal cancer: guaiac FOBT (gFOBT); the fecal immunochemical (or immunohistochemical) test (FIT, also known as iFOBT); and multitargeted stool DNA testing (also known as FIT-DNA).
Virtual colonoscopy is also known as screening CT colonography. Unlike traditional colonoscopy, which requires a scope to be inserted into your rectum and advanced through your colon, virtual colonoscopy uses a CT scan to produce hundreds of cross-sectional images of your abdominal organs.
The stool DNA test uses a sample of your stool to look for DNA changes in cells that might indicate the presence of colon cancer or precancerous conditions. The stool DNA test also looks for signs of blood in your stool. For this test, you collect a stool sample at home and send it to a laboratory for testing.
The U.S. Preventive Services Task Force recommends against CRC screening for those over age 85, and recommends individualized decision-making for people ages 76-85, but the Task Force "does not stipulate how the practice might be implemented," the researchers noted.
There's no upper age limit for colon cancer screening. But most medical organizations in the United States agree that the benefits of screening decline after age 75 for most people and there's little evidence to support continuing screening after age 85.
You are age 75 or older.
The U.S. Preventative Services Task Force (USPSTF) has made the recommendation that colonoscopies are no longer the best idea for people over the age of 85.
Split-dosed MiraLAX/Gatorade was an effective, safe, and tolerable option for bowel preparation before colonoscopy in the low-risk patients in this study. MiraLAX/Gatorade appears to be more tolerable than Golytely as a bowel cleansing regimen and was the preferred agent by the patients in this study.
If you think you can't drink your colonoscopy prep, a “split-dose” is a good option because it tends to be more tolerable.
If you are considered average risk for colorectal cancer and are reluctant to get a colonoscopy, you may have the option of using home tests. “Typically, primary care doctors will talk to their patients about colon cancer screening when they turn 45 or 50, depending on which guidelines they follow,” Dr.
Flexible sigmoidoscopy
This test is similar to a colonoscopy but only lets the doctor see the rectum and lower part of the colon (sigmoid and descending colon). Before a flexible sigmoidoscopy, you will need to have a light bowel clean-out, usually with an enema.
Is there anyone who should not have the procedure? Colonoscopy is not recommended in pregnant patients, patients 75 years or older, patients with limited life expectancy, or in patients with severe medical problems making them high risk for sedation.
Cardiopulmonary complications are the most common adverse events and the incidence is related to the level of sedation, presence of comorbidities, and prolonged procedure time. In addition, both perforation and bleeding are reported to be more common in the elderly11,12).
The researchers found 3.4% of all patients experienced complications within 30 days of their colonoscopy, while 6.8% of patients who were 75 and older experienced complications. 1 Those complications included gastrointestinal tract hemorrhage, cardiovascular complications, sepsis, and bowel perforation.
Colonoscopy and other invasive testing options are considered safe, but the risks of complications of the bowel preparation, the procedure, and sedation medications are all increased in older patients.
Colonoscopies. Medicare covers screening colonoscopies once every 24 months if you're at high risk for colorectal cancer. If you aren't at high risk, Medicare covers the test once every 120 months, or 48 months after a previous flexible sigmoidoscopy. There's no minimum age requirement.
They concluded that gastrointestinal endoscopy in the very elderly is an extremely safe procedure. Comparatively, our results show no procedure-related mortality, non-existent perforation, and a slightly higher rate of cardiopulmonary complications (hypoxemia, 3.5 %; hypotension, 0.8 %).
Colonoscopy was considered complete in 8862 patients (90.1%), and 8071 (82.1%) had an excellent or good bowel preparation quality. In the cohort, 5649 patients (57.5%) had an estimated life expectancy of 10 or more years, 3443 (35.0%) of 5 to less than 10 years, and 739 (7.5%) of less than 5 years (Figure 2).

You just let it go like you normally would. However, there are some reasons this option isn't as comprehensive as a colonoscopy. Mail-in stool tests have a 92% sensitivity rate for detecting colon cancer, which is on par with a colonoscopy, but only a 17% sensitivity rate for detecting cancer-causing polyps.
