Stay away from rough sports or other situations where you could be bruised, cut, or injured. Brush and floss your teeth gently. Be careful when using sharp objects, including razors and fingernail clippers. Enoxaparin may cause bleeding problems.
Check the patient's most recent lab results, including her complete blood cell and platelet count. Check her medication administration record for previous dose and site. Perform hand hygiene. Prepare the medication and identify the patient according to your facility's guidelines.

Clexane is an anticoagulant that belongs to a group of medicines called Low Molecular Weight Heparin (LMWH). These medicines help to prevent clots from getting bigger and stopping new clots from forming. Clexane is used to prevent clotting following hospital procedures or illness.
Do not drink cranberry juice, grapefruit juice or pomegranate juice while you're taking warfarin. They can increase the blood-thinning effect of warfarin. You should also seek advice before taking supplements containing vitamin K. The effect of warfarin is also affected by alcohol.
NURSING IMPLICATIONS
Monitor platelet count closely. Withhold drug and notify physician if platelet count less than 100,000/mm3. Monitor closely patients with renal insufficiency and older adults who are at higher risk for thrombocytopenia. Monitor for and report immediately any sign or symptom of unexplained bleeding.

The antidote for LMWH is Protamine sulphate. This reverses most, but not all, of the effects of LMWH. The dose of protamine sulphate given is dependent upon the dose of LMWH administered and the time of administration.
Report chest pain; persistent constipation; persistent erection; unusual bleeding or bruising (bleeding gums, nosebleed, blood in urine, dark stool); pain in joints or back; or numbness, tingling, swelling, or pain at injection site. Symptoms of Clexane 80 MG Injection overdose include hemorrhage.

Choosing the site for administration:
Direct subcutaneous injection should be given into a subcutaneous tissue skinfold of the abdomen or the upper-outer aspect of the thigh. The skinfold should be held throughout the injection.
