Doma is devoid of human feelings
Even when he was a human, he had little understanding of emotions. He felt no emotion as a child when his parents died violent deaths. As a result, everything he does appears to be a staged performance. Even the Demon King Muzan despises Doma.

Douma said that he had never felt emotions before, but his heart felt like it was beating now. He started doubting his belief that heaven and hell didn't exist and started blushing at Shinobu, quickly developing a crush on her.
DÅ ma is unabashedly masochistic and even sometimes pokes holes in his skull with his own finger to fiddle his brain and trigger some memories. There's also that one instance when he gleefully anticipates Muzan's punishments after a failure since he enjoys physical abuse.
