Kokushibo likely held contempt for Douma due to their contrasting personalities and Kokushibo's deep-seated envy, seeing Douma's effortless power and carefree, emotionless nature as an affront to his own struggle for strength and acknowledgment, even while respecting Douma's rank as Upper Two. While Kokushibo focused on surpassing his brother Yoriichi through immense effort and bitterness, Douma, a natural sociopath, felt no real emotion, which would have deeply bothered the intense Kokushibo.
Kokushibo doesn't trust DOMA the same way Muzan doesn't. because of how fake he is. Not wanting to disrespect Muzan, he ignores him though, only interfering when necessary, like at the first Upper Moon meeting.
