Her frailty and innocence work against her as she cannot cope with the unfolding of one traumatic event after another. Ophelia's darling Hamlet causes all her emotional pain throughout the play, and when his hate is responsible for her father's death, she has endured all that she is capable of enduring and goes insane.

Their particular form of madness was more related to hysteria -- an affliction which was considered to be particularly feminine. Clinically speaking, Ophelia's behavior and appearance are characteristic of the malady the Elizabethans would have diagnosed as female love-melancholy, or erotomania.

Summary and Analysis Act III: Scene 1. Aware that they are being watched, Hamlet stages his own response and argues that he gave her nothing and that he has never loved her. He tells her to go to a nunnery, assaulting her with another double entendre insult.
Hamlet acts mad in order to manipulate Ophelia into believing he is gone crazy. Hamlet planned to create a dominos effect that would start with Ophelia thinking he has gone mad which would lead her to pass this information onto Polonius further passing it onto Claudius.
Ophelia cannot trust her beloved father or her lover, Hamlet, because they are all simply using her against each other. By this logic, of course she went insane.
Bidding his sister, Ophelia, farewell, he cautions her against falling in love with Hamlet, who is, according to Laertes, too far above her by birth to be able to love her honorably. Since Hamlet is responsible not only for his own feelings but for his position in the state, it may be impossible for him to marry her.
Hamlet's rejection allowed him to reject Ophelia and his love for her. Although he does love her, he couldn't let Ophelia distract him from getting his revenge. Hamlet separates himself away from his family and friends and harrasses Ophelia by naming her vulgar comments.
In this way, Ophelia's drowning, surrounded by flowers with her last breath in song, is her surrender to her overwhelming femininity. In an ironic twist, her watery suicide, which plunges her into the impersonal eternal female essence, works to remind the characters of her humanity and individuality.
Ophelia's final words are addressed to either Hamlet, or her father, or even herself and her lost innocence: “And will a not come again? / No, no, he is dead, / Go to thy death-bed, / He never will come again. / … / God a mercy on his soul. And of all Christian souls. God buy you.” Next, she drowns herself.
Ophelia's tragic flaw is the loyalty she affords those she loves. This loyalty renders her incapable of emotionally handling his mistreatment, in addition to the demands of her father and brother.
However, the explicit sexual references in Ophelia's songs perhaps account for her obsession with the now absent Hamlet, as in “promising his love” to her earlier in the play and then being scorned, she is doubly heartbroken alongside the death of her father.

Instead they are referred to in a conversation between Queen Gertrude and Ophelia's brother Laertes. Gertrude describes how Ophelia fell into the river while picking flowers and slowly drowned, singing all the while.
Turns out Ophelia's mom was killed thanks to "being close to the royal family" (Corgi attack, probably), which caused her dad to shut down emotionally.
