Immunotherapy following surgery for advanced melanoma
Adjuvant therapy is additional cancer treatment given after primary surgical treatment. Nivolumab, ipilimumab, and pembrolizumab have all been approved as adjuvant therapies for melanoma that has spread to nearby lymph nodes but can be removed with surgery.
In a groundbreaking advance in March 2022 led by Hopkins scientists and oncologists, the FDA approved the combination of anti-PD-1 with another checkpoint blocker, anti-LAG-3, as the first systemic treatment a patient might receive for advanced melanoma.
Surgery to remove the tumor is the primary treatment of all stages of melanoma. A wide local excision is used to remove the melanoma and some of the normal tissue around it. Skin grafting (taking skin from another part of the body to replace the skin that is removed) may be done to cover the wound caused by surgery.
However, in early stage cases, early diagnosis allows for the opportunity to improve the chances of survival with a prompt surgery. Although many physicians treat melanoma surgically three to four weeks after diagnosis, there is no official recommendation on time to treatment.
Stage IV melanoma is difficult to cure as it has already spread to other parts of the body. However, a growing number of people respond well to treatment, achieve No Evidence of Disease (NED), and survive for many years.
How fast does melanoma spread and grow to local lymph nodes and other organs? “Melanoma can grow extremely quickly and can become life-threatening in as little as six weeks,” noted Dr. Duncanson.
Nodular melanoma skin cancer has a poor prognosis because it grows down into the skin (vertical growth pattern) and tends to be thick when diagnosed.

They can grow quite quickly. There is often a raised area on the skin surface with this type of melanoma.
It remains a tough cancer to beat. But advances in areas such as immunotherapy and targeted therapy have transformed the treatment of melanoma, offering tremendous hope. Even patients with the most severe cases are prolonging their lives by months or years, and some are even going into long-term remission.

The 5-year survival rate is around 15% to 20%. The 10-year survival rate is 10% to 15%. This rate is higher if the cancer has spread only to the skin or distant lymph nodes and not to vital organs.
Targeting gene changes can stop the cancer from growing and spreading. Medications that can do this include dabrafenib (Tafinlar), trametinib (Mekinist), and vemurafenib (Zelboraf).
Melanoma treatment can often remove the cancer. Caught early, the disease has a nearly 100 percent cure rate. But melanoma can come back.
Choose a variety of colorful fruits and vegetables to get the greatest benefit. Aim to eat a minimum of 5 servings of whole fruits and vegetables daily. Choose sources of healthy fat. Avoid fried, greasy, and fatty foods, Choose baked, broiled, or grilled foods instead.
Normally, the first place a melanoma tumor metastasizes to is the lymph nodes, by literally draining melanoma cells into the lymphatic fluid, which carries the melanoma cells through the lymphatic channels to the nearest lymph node basin.
Nodular melanoma
This is the most aggressive form of cutaneous melanoma. Nodular melanoma typically appears as a dark bump—usually black, but lesions may also appear as colorless skin tones.
Symptoms if cancer has spread to the lymph nodes
The most common symptom if cancer has spread to the lymph nodes is that they feel hard or swollen. Swollen lymph nodes in the neck area can make it hard to swallow. Cancer cells can also stop lymph fluid from draining away.
If your melanoma has spread to other areas, you may have: Hardened lumps under your skin. Swollen or painful lymph nodes. Trouble breathing, or a cough that doesn't go away.
While most melanomas are detected at an early stage, a proportion of patients have metastatic disease at the time of diagnosis or develop metastasis at a later stage. The most common sites of metastasis are skin and subcutaneous tissue, followed by lungs, liver, bones, and brain.
How long can you have melanoma and not know it? It depends on the type of melanoma. For example, nodular melanoma grows rapidly over a matter of weeks, while a radial melanoma can slowly spread over the span of a decade. Like a cavity, a melanoma may grow for years before producing any significant symptoms.
Melanoma is often considered a disease of older people, since about half of melanomas occur in people over the age of 50, with the median age at diagnosis being 59. While older adults are at higher risk of developing melanoma, the incidence of melanoma is rapidly rising in young adults.
These test results along with the results from your skin biopsy, complete skin exam, and physical are used to determine the stage of the melanoma. When everything that your doctor sees suggests that the cancer may have spread to a lymph node, your doctor may recommend a procedure called a sentinel lymph node biopsy.
