Heartburn, angina and heart attack may feel very much alike. Even experienced doctors can't always tell the difference from your medical history and a physical exam. That's why, if you go to the emergency room because of chest pain, you'll immediately have tests to rule out a heart attack.
Heart-related chest pain
Crushing or searing pain that spreads to your back, neck, jaw, shoulders, and one or both arms. Pain that lasts more than a few minutes, gets worse with activity, goes away and comes back, or varies in intensity. Shortness of breath. Cold sweats.
Chest pain from a heart attack often feels like a large amount of pressure, tightness, burning, or squeezing in the chest. In comparison, chest pain that feels like a sharp or knife-like pain resulting from coughing or breathing is likely not due to a heart attack.
Most heart attacks involve discomfort in the center or left side of the chest that lasts for more than a few minutes or that goes away and comes back. The discomfort can feel like uncomfortable pressure, squeezing, fullness, or pain. Feeling weak, light-headed, or faint.
Symptoms that suggest another problem
More often than not, chest pain does not signal a heart attack. A study of emergency room visits found that less than 6% of patients arriving with chest pain had a life-threatening heart issue.
Noncardiac chest pain is defined as recurring pain in your chest — typically, behind your breast bone and near your heart — that is not related to your heart. In most people, noncardiac chest pain is actually related to a problem with their esophagus, most often gastroesophageal reflux disease (GERD).
The pain of a heart attack differs from that of a strained chest muscle. A heart attack may cause a dull pain or an uncomfortable feeling of pressure in the chest. Usually, the pain begins in the center of the chest, and it may radiate outward to one or both arms, the back, neck, jaw, or stomach.
Angina is chest pain or discomfort caused when your heart muscle doesn't get enough oxygen-rich blood. It may feel like pressure or squeezing in your chest. The discomfort also can occur in your shoulders, arms, neck, jaw, abdomen or back. Angina pain may even feel like indigestion.

It could be a lung disorder, such as a blood clot to the lungs, known as a pulmonary embolism. Additionally, other causes of chest discomfort include spasm of the esophagus, diseases of the aorta, gastroesophageal reflux disease, musculoskeletal pain, fast heart rhythm abnormalities and costochondritis.
Chest pain and heart problems
The most common heart problems that cause chest pain include: pericarditis – which usually causes a sudden, sharp, stabbing pain that gets worse when you breathe deeply or lie down. angina or a heart attack – which have similar symptoms but a heart attack is life-threatening.

Sharp, shooting, or stabbing pain. Persistent, dull aching. Tightness, tension, or pressure. A burning sensation.
Typical heart attack symptoms
This discomfort or pain can feel like a tight ache, pressure, fullness or squeezing in the chest lasting more than a few minutes. This discomfort may come and go.
Changing hormone levels can cause changes in the milk ducts or milk glands. These changes in the ducts and glands can cause breast cysts, which can be painful and are a common cause of cyclic breast pain. Noncyclic breast pain may be caused by trauma, prior breast surgery or other factors.
Heart pain is usually not sharp or stabbing. In general, any pain that gets worse with movement is not from the heart. Chest pain that is aching, sharp or stabbing, may be caused by other conditions such as acid reflux or heartburn, pleurisy or joint and muscle pain.
Chest pain can stem from many health issues. Some are quite serious, while others may be nothing to worry about. Sometimes, chest pain indicates a blocked artery and a heart attack. This is an emergency situation, in which the heart is not receiving enough blood and oxygen to function correctly.
Call 911 or have someone take you to the closest emergency room right away if you have chest pain that lasts longer than five minutes and doesn't go away when you rest or take medication. Cardiac chest pain can be life-threatening. Chest pain can be a sign of a heart attack.
Red flags to alert a possible diagnosis of heart attack
Clammy, unwell patient. Exertional chest pain. Heavy, tight, pressure type chest pain. Pain radiating to left arm, right shoulder or both arms.
You may have a perfectly normal ECG, yet still have a heart condition. If your test is normal but your doctor suspects that you have a heart problem, he may recommend that you have another ECG, or a different type of test to find out for sure.
An EKG Has Limits
It measures heart rate and rhythm—but it doesn't necessarily show blockages in the arteries unless they are causing acute loss of blood flow to the heart muscle.
While anxiety chest pain varies from person to person, you may feel a constant, mild sense of discomfort or chest pain that comes and goes throughout the day. Panic attacks can last anywhere from five to 30 minutes.
