Tommy finds out he's not dying from a vision of his deceased daughter, Ruby, in the Peaky Blinders Season 6 finale, revealing his terminal tuberculoma diagnosis was a lie orchestrated by Oswald Mosley and his ally, Dr. Holford, who wanted Tommy to kill himself, but instead, Tommy spares the doctor and rides off for a new life.
What happens to Tommy's brain tumor? Well, he thought he had a brain tumor. But once he discovers that the doctor who diagnosed him is friends with Tommy's onetime fascist political partner, Oswald Mosley (Sam Claflin), he realizes it's all a ruse, and he isn't sick at all.
Laura McKee informs Thomas that the IRA are the ones who foiled his assassination attempt of Mosley. The IRA want to protect Mosley's future in British and Irish politics. Diana and Oswald get married at the home of Joseph Gobbels, with Adolf Hitler as guest of honor.
John Shelby dies in Peaky Blinders season 4, and it's easily one of the most heartbreaking deaths in the show. Of all the Shelby brothers, John (Joe Cole) had the biggest family and took the best care of them.

Grace Shelby
Although Grace is completely loyal to Tommy, she is distrusted by Polly and Arthur. His love for Grace makes Tommy stronger, but also more vulnerable. She is the only thing he still truly cares about and enemies can hurt him.
She was killed by the IRA, as a consequence for her nephews' actions after they tried to have Oswald Mosley assassinated. Her body was then delivered to Thomas' house, Arrow House, where he broke down in tears from grief and guilt.
Treating PTSD
It's easy to see why people like Tommy, who experience PTSD due to traumatic events might turn to alcohol or drugs. But this increases mental health problems because it exacerbates them and can lead to severe anxiety, panic, depression, and suicidal ideation – as seen with Tommy.
Grace Shelby (née Burgess) was Tommy's first love in Peaky Blinders, but show creator Steven Knight says the story demanded her death.

Throughout Peaky Blinders, he's rarely shown consuming food. It's not an oversight—it's intentional. His character is driven by ambition, trauma, and control. Eating would suggest comfort, and Tommy doesn't allow himself that luxury.

As Thomas's eldest son and new member of the Peaky Blinders, it is expected that he will inherit the leadership of the gang some day.
Following Ruby's diagnosis, both Tommy and Lizzie were examined for signs of the disease, but unlike Lizzie, who was given the all-clear, Tommy's fate looks bleak. The doctor informed the head of the Shelby clan that he has tuberculoma, a non-cancerous growth or tumour brought on by his daughter's illness.
Helen McCrory left Peaky Blinders because she passed away from breast cancer in April 2021, preventing her return for the sixth and final season; her character, Aunt Polly Gray, was written out and killed off in the storyline to reflect the real-life loss, impacting the show profoundly.
