Poor patient selection, poor understanding of facial anatomy, poor placement, and poor product selection can make the under eye area a very challenging area to treat well. Done correctly, it can make a HUGE difference in a patient's overall look.
Although more significant complications with under eye facial filler injections (tear trough rejuvenation) are rare overall; unfortunately, they can occur, as with any facial filler procedure, even if the injection was performed by a qualified and skilled injector.
You may not be an ideal candidate for tear trough filler if you have: Very thin skin. Dark circles caused by lifestyle habits or genetics. Medical conditions that add a risk of complications.
Eye fillers can work wonders, but they're not a miracle solution. For example, they're not permanent, and they won't address some concerns, such as crow's feet. Talking to a doctor about the results you're hoping for is an important first step. Everyone deserves to feel confident about their looks.
According to the American Society of Plastic Surgeons, you can expect some bruising, since the under-eye area has thinner skin with lots of blood vessels. Swelling is common as well. There are other possible side effects: If the fillers aren't injected correctly, the area can end up looking more swollen and discolored.
If you're looking to a dermal filler to combat signs of aging, your mid-20s is often a good time to start. Your body starts to lose bone and collagen around age 26, so it's a good time to begin maintenance injections. By starting early, you'll use require less product than if you wait until your mid-50s.

Fillers are a great option for patients seeking a softer, more youthful look. However, if used improperly or over used, fillers can have negative long term consequences. In fact, patients who do not properly use filler could actually speed up their skin's aging process, resulting in older looking skin.
If you have puffiness
“In some patients, they have depleted fat pads, and they require more volume from filler to fill the crevice,” explains Dr Ewoma. “However, a lot of people have fat pads that are bulging, and are too big and baggy. When that is the case, filler is absolutely going to make it worse.”
But in general, the best candidates for under eye filler are patients who have true hollows or depressions under the eye, and also relatively good skin quality and tightness in the area.
“The short answer is no,” says Dr. Miriam Hanson, board certified dermatologist and cosmetic expert in Austin, Texas. “Wrinkles do not become worse after having dermal fillers.” Dermal fillers comprise a family of injectable medications that restore volume in areas of the skin where it has been lost.
The tear trough area is hard to treat because due to the thin skin under your eyes. This area also has less fat to cover any fillers that may are used in that area.
There are also occasions where patients who get fillers frequently do not respond as well to the same filler since the injected tissues start to develop channels, swelling or firmness. For instance, Radiesse can form tissue tracks after as little as 3 serial treatments.
Answer: Laugh lines worse after Filler
This may appear worse because there is swelling from the injection. All swelling should subside in about 2 weeks. If after 2 weeks this does not get better, then please see your injector for an evaluation.
Although extremely rare, dermal fillers are able to migrate within the skin if consistent and adequate pressure is applied to them. For this reason, we advise patients not to get a facial, massage (on the treated skin), or microdermabrasion for the first two weeks following their filler injections.
Issues in this area are often associated with looking tired. Non-surgically, tear troughs can be treated with dermal fillers injection in the mid-cheek to support the lower eyelid. Patients generally require 0.5ml to 1ml of tear trough filler to get a fresh-faced look.
Botox and dermal fillers work differently which is why they're best used in specific parts of the face. A filler is great for restoring facial volume under the eyes, while Botox is effective for smoothening the dynamic wrinkles like glabellar lines and crow's feet.
If it is determined that tear trough filler is the right treatment is right for you, the filler injection will be applied. You should avoid drinking alcohol for 24 hours before the treatment, whilst aspirin should not be taken for 7 days and Ibuprofen for two days / 48 hours.

The tear trough which is lower on the eyelid skin adjacent to the beginning of the top of the cheek, can be filled below the muscle and gives a more natural appearance. It won't freeze your face when you smile because you should not get such a large volume that it interferes with your smile or lumps up.
Once under eye filler wears off it will not cause more wrinkles, but your skin will resume the appearance it had prior to the filler injections.
