In The Boys comics, the original Black Noir (Mallory) actually defeats Soldier Boy by not fighting him directly, but by having the other members of The Boys (Butcher, MM, Hughie, MM's dad) overwhelm and kill him, leveraging Soldier Boy's rage and arrogance, while Black Noir himself is more of a quiet, mysterious operative, not a direct powerhouse like Soldier Boy in the show, but a sneaky, deadly one.
Before Mindstorm and Crimson Countess could subdue him, Soldier Boy pinned Black Noir's face to the hood of a burning Jeep and bashed his head in with his shield, severely damaging and even removing a part of Black Noir's brain, leaving him brain damaged and scarred forever.

Soldier Boy is a gritty parody of Captain America. He's a racist tool of the government who used to turn hoses on protesters. I can understand why people might claim he's a "real-world" version of Cap. A cynical person could believe that a real-world supersoldier would probably not have Cap's optimism and morality.
Despite being referred to as a "brain tumor", Kessler in actuality seems to be a worm-esque parasite existing under Butcher's skin, generally around the brain region. Homelander refers to it as a black mass swirling around his brain, implying the existence of many worms that function as a single entity.
